摘  要:目的对肝细胞癌进行免疫分子分型,研究其微环境特征和功能,筛选与预后相关的基因,构建预后模型,探索其临床、免疫和治疗特性。方法使用单样本基因集富集分析(single sample gene set enrichment analysis,ssGSEA)方法,对肝细胞癌进行免疫途径评分,并基于此进行层次聚类分析;对免疫亚型进行差异表达基因分析,并结合ImmPort数据库下载的免疫基因确定免疫相关差异表达基因;使用单变量Cox回归和LASSO回归分析,识别预后相关基因,建立免疫风险评分预后模型,并分析模型的预后价值、免疫浸润特征、临床特征和药物敏感性。结果通过ssGSEA算法,识别出高免疫组和低免疫组,并筛选出1433个差异表达基因。与ImmPort数据库交集后,确定265个免疫相关差异表达基因。通过Cox和Lasso回归分析,确定16个预后相关基因,构建风险预后模型。Kaplan-Meier分析显示低风险组的总生存期明显优于高风险组,模型预测1年、3年、5年总生存率的AUC在训练集和验证集中表现良好。结论本研究确定两种免疫分型,构建基于免疫预后基因的HCC风险预后模型。该模型能够评估肝细胞癌(hepatocellular carcinoma,HCC)的免疫微环境,明确肿瘤免疫表型,并指导临床实践和个体化免疫治疗。Objective This study aims to perform immune molecular subtyping of hepatocellular carcinoma(HCC),investigate its microenvironment characteristics and functions,screen for prognostic-related genes,construct a prognostic model,and explore its clinical,immune,and therapeutic characteristics.Methods The single sample Gene Set Enrichment Analysis(ssGSEA)method was used to score immune pathways in HCC,followed by hierarchical clustering analysis based on these scores.Differentially expressed genes of immune subtypes were analyzed,and immune-related differentially expressed genes were identified according to immune genes downloaded from ImmPort database.Univariate Cox regression and Lasso regression analyses were performed to identify prognostic-related genes and construct an immune risk score prognostic model.The model′s prognostic value,immune infiltration characteristics,clinical characteristics,and drug sensitivity were analyzed.Results Using the ssGSEA algorithm,high immune and low immune groups were identified,and 1433 differentially expressed genes were screened.Cross-referencing with the ImmPort database identified 265 immune-related differentially expressed genes.Through Cox and Lasso regression analyses,16 prognostic-related genes were identified,and a risk prognostic model was constructed.Kaplan-Meier analysis showed that the overall survival of the low-risk group was significantly better than that of the high-risk group.The model′s area under the curve(AUC)for predicting 1-year,3-year,and 5-year overall survival performed well in both the training and validation sets.Conclusion This study identified two immune subtypes and constructed an HCC risk prognostic model based on immune prognostic genes.This model can evaluate the immune microenvironment of HCC,clarify tumor immune phenotypes,and guide clinical practice and personalized immunotherapy.
