针灸联合超声引导下颈2神经根阻滞治疗颈源性头痛的临床效果

摘　　要：目的 观察针灸联合超声引导下颈2(C2)神经根阻滞治疗颈源性头痛的有效性。方法 选取2020年1月至2022年10月于新乡医学院第一附属医院疼痛科诊治的72例患者作为观察对象,随机分为治疗组与对照组,各36例。对照组接受超声引导下C2神经根阻滞治疗。治疗组在行C2神经根阻滞的同时复合针灸治疗。观察治疗前和治疗后1周、1个月、3个月、6个月两组疼痛程度[数字评分法(NRS)评分]、睡眠质量[匹兹堡睡眠质量指数(PSQI)评分]、颈椎活动度和临床疗效的差异。结果 治疗后1周、1个月、3个月、6个月,两组NRS及PSQI评分较治疗前均降低(P<0.05),且治疗组NRS、PSQI评分均低于对照组(P<0.05),治疗后6个月两组颈椎活动度较治疗前均改善,治疗组颈椎活动度改善优于对照组(P<0.05),治疗组总有效率高于对照组(P<0.05)。结论 针灸联合超声引导下C2神经根阻滞治疗颈源性头痛,能有效减轻头痛、改善睡眠质量及提升颈椎活动度,临床疗效显著。Objective To observe the clinical efficacy of acupuncture combined with ultrasound-guided cervical 2(C 2)nerve root block in the treatment of cervical headache.Methods A total of 72 patients in Pain Department of the First Affiliated Hospital of Xinxiang Medical College from January 2020 to October 2022 were randomly divided into treatment group and control group,with 36 cases in each group.The control group was treated with ultrasound-guided C 2 nerve root block.The treatment group was treated with C 2 nerve root block combined with acupuncture.The pain degree[numerical rating scale(NRS)score],sleep quality index[Pittsburgh sleep quality index(PSQI)score],range of motion and clinical efficacy of the two groups were observed before treatment,and 1 week,1 month,3 months and 6 months after treatment.Results The NRS score and PSQI score of both groups were lower than those before treatment at 1 week,1 month,3 months and 6 months after treatment(P<0.05),and the NRS score and PSQI score of the treatment group were lower than those of the control group(P<0.05).The range of motion of both groups at 6 months after treatment were improved than those before treatment(P<0.05),and the range of motion of the treatment group was better than that of the control group(P<0.05).The total effective rate of the treatment group was higher than that of the control group(P<0.05).Conclusion Acupuncture combined with ultrasound-guided C 2 nerve root block in the treatment of cervical headache has a significant effect in reducing the degree of headache,improving sleep quality and enhancing cervical motion.
