摘 要:目的 对比Bio-Oss/富血小板纤维蛋白(PRF)复合物和Bio-Oss骨粉在牙槽骨缺损区行正畸矫治患者中的应用效果。方法 将2022年3月至2024年3月濮阳市第三人民医院收治的78例由于牙槽骨缺损而进行正畸矫治的患者作为研究对象,78例患者中共133颗牙,将所有患者分为对照组(39例,65颗牙)和研究组(39例,68颗牙)。对照组患者仅将Bio-Oss骨粉植入牙槽骨缺损区,研究组患者则植入Bio-Oss/PRF复合物。将两组患者创面愈合情况、不同时间点正畸牙齿垂直骨吸收量级骨密度平均值、手术前后炎症因子相关指标水平以及术后并发症发生情况进行对比。结果 研究组患者愈合时间比对照组短,而术后2周创面愈合率相较于对照组上升(P<0.05);研究组患者不同时间点垂直骨吸收量低于对照组,而骨密度平均值高于对照组(P<0.05);术后3个月两组患者白介素-4(IL-4)、肿瘤坏死因子-α(TNF-α)、γ-干扰素(IFN-γ)水平均比术前下降,且研究组低于对照组(P<0.05);两组患者并发症发生情况经对比,差异无统计学意义(P>0.05)。结论 Bio-Oss/PRF复合物应用于牙槽骨缺损区行正畸矫治患者中,可促进创面愈合,有助于骨生成,提升骨平均密度,减轻炎症反应,且安全性良好。Objective To compare the effect of Bio-Oss/platelet-rich fibrin(PRF)complex and Bio-Oss bone meal in orthodontic treatment of alveolar bone defect.Methods A total of 78 patients admitted to the Third People’s Hospital of Puyang from March 2022 to March 2024 who underwent orthodontic treatment due to alveolar bone defect were selected as the study objects.The 78 patients had 133 teeth in total.All patients were divided into control group(39 cases,65 teeth)and study group(39 cases,68 teeth).Only Bio-Oss bone meal was implanted into the alveolar bone defect in the control group,while Bio-Oss/PRF complex was implanted in the study group.Wound healing,the average bone mineral density of orthodontic teeth at different time points,the level of inflammatory factors related indexes before and after surgery,and the incidence of postoperative complications were compared between the two groups.Results The healing time of the study group was shorter than that of the control group,and the wound healing rate of 2 weeks after surgery was increased(P<0.05).The vertical bone absorption in the study group was lower than that in the control group at different time points,and the mean bone mineral density was higher than that in the control group(P<0.05).The levels of interleukin-4(IL-4),tumour necrosis factor-α(TNF-α)and interferon-γ(IFN-γ)were decreased in both groups 3 months after surgery,and the study group was lower than the control group(P<0.05).There was no difference in complications between the two groups(P>0.05).Conclusion Bio-Oss/PRF compound can promote wound healing,contribute to bone formation,increase mean bone density and reduce inflammation in patients with alveolar bone defect treated with orthodontic treatment,and has good safety.
