双主刀模式下腹腔镜中肝区域肝切除术的疗效分析

摘　　要：目的分析双主刀模式下腹腔镜中肝区域肝切除术的临床疗效。方法回顾性分析2021年4月至2022年7月柳州市人民医院肝胆胰外科收治的56例腹腔镜中肝区域肝切除术患者的临床资料,根据不同手术方法分为对照组与观察组。观察组采取双主刀模式下腹腔镜中肝区域肝切除术,对照组采取传统单主刀模式下腹腔镜中肝区域肝切除术。比较两组手术时间、手术出血量、中转开腹手术率、术后并发症发生率。结果观察组手术时间短于对照组,手术出血量少于对照组,中转开腹手术率低于对照组,差异有统计学意义(P<0.05);两组术后并发症发生率比较差异无统计学意义。结论双主刀模式下腹腔镜中肝区域肝切除术可缩短手术时间,减少患者手术出血量,降低中转开腹手术率,且安全性较高。Objective To analyze the clinical efficacy of laparoscopic mid-hepatic regional hepatectomy under the dual-surgeon mode.Methods The clinical data of 56 patients who underwent laparoscopic mid-hepatic region hepatectomy in the department of Hepatobiliary and Pancreatic Surgery in Liuzhou People's Hospital from April 2021 to July 2022 were retrospectively analyzed,they were divided into the control group and the observation group according to different surgical methods,with 28 cases in each group.The observation group was treated with the laparoscopic mid-hepatic regional hepatectomy under the dual-surgeon mode,while the control group was treated with the laparoscopic mid-hepatic regional hepatectomy under traditional single-surgeon mode,with 28 cases in each group.The operation time,amount of surgical bleeding,conversion to open surgery rate,and postoperative complication rate were compared between the two groups.Results The operation time in the observation group was shorter than that in the control group,the amount of surgical bleeding was less than that in the control group,and conversion to open surgery rate was lower than that in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in postoperative complications between the two group.Conclusion Laparoscopic mid-hepatic region hepatectomy under the dual-surgeon mode can shorten the operation time,reduce the amount of surgical bleeding,and reduce the conversion rate of open surgery,and has high safety.
