阿替普酶静脉溶栓辅助治疗急性重症脑梗死的临床价值

摘　　要：目的探讨阿替普酶静脉溶栓辅助治疗急性重症脑梗死患者的临床价值。方法选取2019年1月至2021年1月于沈阳市第九人民医院接受治疗的78例急性重症脑梗死患者作为研究对象,采取随机数字表法分为对照组与观察组,每组39例。对照组行常规治疗,观察组在对照组基础上给予阿替普酶静脉溶栓辅助治疗,比较两组美国国立卫生研究院卒中量表(National Institute of health stroke scale,NIHSS)评分、临床疗效及不良反应发生情况。结果治疗后12 h、1周、2周,观察组NIHSS评分均明显低于对照组(P<0.05);观察组治疗总有效率为84.62%,明显高于对照组61.54%,差异有统计学意义(P<0.05);观察组不良反应发生率为10.26%,对照组为2.56%,两组比较差异无统计学意义。结论阿替普酶静脉溶栓辅助治疗急性重症脑梗死患者疗效显著,能明显改善患者神经功能,且安全性较高,值得临床推广应用。Objective To explore the clinical value of alteplase intravenous thrombolysis in the adjuvant treatment of patients with acute severe cerebral infarction.Methods 78 patients with acute severe cerebral infarction who were treated in Shenyang Ninth People's Hospital from January 2019 to January 2021 were selected as the research subjects,and they were divided into the control group and the observation group according to random number table method,with 39 cases in each group.The control group received routine treatment,and the observation group received alteplase intravenous thrombolytic adjuvant therapy on the basis of the control group,the National Institute of health stroke scale(NIHSS)scores,clinical efficacy,and incidence of adverse reactions were compared between the two groups.Results After 12 h,1 week,2 weeks of treatment,the NIHSS score in the observation group was significantly lower than those in the control group(P<0.05);the total effective rate in the observation group was 84.62%,which was higher than 61.54%in the control group,the difference was statistically significant(P<0.05);the incidence of adverse reactions in the observation group was 10.26%,while in the control group was 2.56%,there was no statistically significant difference between the two groups.Conclusion Alteplase intravenous thrombolysis assisted treatment of acute severe cerebral infarction patients has significant efficacy,can significantly improve the neurological function of patients,and has high safety,which is worthy of clinical promotion and application.
