血清AGR联合CEA、CA199、CRP对早期直肠癌的诊断价值

摘　　要：目的:探讨血清白蛋白球蛋白比值(AGR)、癌胚抗原(CEA)、糖类抗原19-9(CA199)和C-反应蛋白(CRP)联合检测对早期直肠癌的诊断价值。方法:选取我院2020年6月至2023年12月收治的78例直肠癌患者为观察组,同期选取70名健康志愿者为对照组。使用相应仪器测定患者血清AGR、CEA、CA199和CRP水平以及联合检测,并与对照组进行比较分析。结果:观察组患者血清CEA、CA199和CRP水平显著高于对照组(P<0.05),AGR水平低于对照组(P<0.05)。ROC曲线分析表明,CEA和CA199显示出较高的特异度,分别为95.7%和97.1%。联合检测,灵敏度和特异度显著提高,分别达到88.5%和97.1%。血清AGR、CEA、CA199和CRP的AUC分别为0.672、0.849、0.829和0.841,而联合检测AUC达到0.977,显示出极高的诊断效能。结论:血清AGR联合CEA、CA199和CRP的检测,可以显著提高早期直肠癌的诊断准确性,有助于临床医师在早期阶段识别并治疗直肠癌患者。Objective To investigate the diagnostic value of combined detection of serum albumin globulin ratio(AGR),carcinoembryonic antigen(CEA),carbohydrate antigen 19-9(CA199)and C-reactive protein(CRP)for early rectal cancer.Methods A total of 78 patients with rectal cancer admitted to our hospital from June 2020 to December 2023 were selected as the observation group,and 70 healthy volunteers were selected as the control group during the same period.The serum levels of AGR,CEA,CA199,CRP and the combined detection were measured by the corresponding instruments,and compared with the control group.Results The serum levels of CEA,CA199 and CRP in the observation group were significantly higher than those in the control group(P<0.05),and the AGR level was lower than that in the control group(P<0.05).ROC curve analysis showed that CEA and CA199 indicated higher specificity,which were 95.7%and 97.1%,respectively.The sensitivity and specificity of combined detection were significantly improved,reaching 88.5%and 97.1%,respectively.The AUC of serum AGR,CEA,CA199 and CRP were 0.672,0.849,0.829 and 0.841,respectively,and the AUC of combined detection reached 0.977,showing a high diagnostic efficiency.Conclusion Serum AGR combined with CEA,CA199 and CRP can significantly improve the diagnostic accuracy of early rectal cancer,and help clinicians to identify and treat patients with rectal cancer in the early stage.
