双能量CT动脉期与静脉期定量参数在结直肠癌诊断中的应用价值

摘　　要：目的:本研究旨在分析双能量CT定量参数在结直肠癌诊断中的应用价值。方法:回顾性分析2021年10月至2023年10月我院收治的疑似结直肠癌患者76例临床资料,所有患者均通过双能量CT行腹部动脉期和静脉期平扫。根据病理学检查结果将患者分为良性组41例和恶性组35例,比较2组患者肿瘤部位的碘浓度(IC)、标准化碘浓度(NIC)、能谱曲线斜率λ_(Hu)。通过Logistic回归分析评估良恶性肿瘤与双能量CT定量参数的关系,并利用ROC曲线分析双能量CT定量参数在评估结直肠癌良恶性中的诊断价值。结果:恶性组患者动脉期和静脉期的IC、NIC和λ_(Hu)均高于良性组,差异均有统计学意义(P<0.05);Logistic分析显示,双能量CT动脉期与静脉期定量参数均是影响肿瘤良恶性的重要因素(P<0.05);ROC曲线显示,双能量CT对结直肠癌诊断有较高的准确度。结论:双能量CT动脉期与静脉期定量参数在结直肠癌诊断中有较高的应用价值。Objective To analyze the application value of quantitative parameters of dual-energy CT in the diagnosis of colorectal cancer.Methods The clinical data of 76 patients with suspected colorectal cancer admitted to our hospital from October 2021 to October 2023 were retrospectively analyzed.All patients underwent abdominal arterial phase and venous phase plain scan by dual-energy CT.According to the results of pathological examination,the patients were divided into the benign group(41 cases)and the malignant group(35 cases).The iodine concentration(IC),normalized iodine concentration(NIC),andλ_(Hu)of the spectral curve were compared between the two groups.Logistic regression analysis was used to evaluate the relationship between the benign and malignant tumors and quantitative parameters of dual-energy CT.ROC curve was used to analyze the diagnostic value of quantitative parameters of dual-energy CT in evaluating the benign and malignant nature of colorectal cancer.Results The IC,NIC andλ_(Hu)of the malignant group in arterial phase and venous phase were higher than those of the benign group,and the differences were statistically significant(P<0.05).Logistic analysis showed that the quantitative parameters of arterial phase and venous phase of dual-energy CT were important factors affecting the benign and malignant tumors(P<0.05).The ROC curve showed that dual-energy CT had high accuracy in the diagnosis of colorectal cancer.Conclusion The quantitative parameters of arterial phase and venous phase of dual-energy CT have high application value in the diagnosis of colorectal cancer.
