摘　　要：目的探讨术前全身炎症反应指数(SIRI)联合纤维蛋白原-白蛋白比值(FAR)分析对结直肠癌患者的临床意义。方法本研究以宣城仁杰医院自2021—2024年收治的158例结直肠癌患者为研究组,以同期106名健康体检者为对照组,统计两组患者外周血实验室检查结果,利用多因素Logistic回归分析结直肠癌发生的危险因素。结果结直肠癌患者术前SIRI和FAR水平均高于对照组(P<0.05),而且SIRI和FAR是结直肠癌发生的危险因素;ROC曲线分析显示SIRI联合FAR对结直肠癌的诊断效能(AUC=0.832,敏感性94.9%,特异度77.9%)高于FAR或SIRI单独使用时;SIRI-FAR评分高的结直肠癌患者与低评分患者相比,CEA水平和BMI值较高、TMN分期较高,且KRAS、NRAS和BRAF基因突变比例明显增高(P<0.05)。结论结直肠癌患者术前SIRI-FAR评分与其临床病理参数密切相关,可作为结直肠癌患者重要的辅助诊断检测指标。Objective To explore the clinical significance of preoperative systemic inflammatory response index(SIRI)combined with fibrinogen albumin ratio(FAR)analysis in patients with colorectal cancer.Methods This study selected 158 patients with colorectal cancer admitted to Xuancheng Renjie hospital from 2021 to 2024 as the research group,and 106 healthy individuals who underwent physical examinations during the same period as the control group.The peripheral blood laboratory test results of the two groups of patients were statistically analyzed,and multiple Logistic regression was used to analyze the risk factors for the occurrence of colorectal cancer.Results The preoperative levels of SIRI and FAR in colorectal cancer patients were higher than those in the control group(P<0.05),and SIRI and FAR were risk factors for the occurrence of colorectal cancer.ROC curve analysis showed that the diagnostic efficacy of SIRI combined with FAR for colorectal cancer(AUC=0.832,sensitivity 94.9%,specificity 77.9%)was higher than that of FAR or SIRI alone.Compared with low score patients,colorectal cancer patients with high SIRI-FAR scores have higher levels of CEA and BMI,higher TMN staging,and significantly higher proportions of KRAS,NRAS,and BRAF gene mutations(P<0.05).Conclusions The preoperative SIRI-FAR score of colorectal cancer patients is closely related to their clinical and pathological parameters,and can be used as an important auxiliary diagnostic test indicator for colorectal cancer patients.
