热敏灸治疗功能性消化不良优化灸治(单点灸)方案的研究

摘　　要：目的观察热敏灸(单点灸)治疗功能性消化不良(FD)的临床疗效,并比较其疗效差异,探讨热敏灸治疗FD的优化灸治(单点灸)方案。方法选取120例符合标准的FD患者,所有病例均来自2020年9月至2021年12月赣州市立医院康复科、中医科及消化内科门诊及住院部。以查找热敏穴的金标准——探感定位法,在FD的高发热敏穴区(中脘、天枢、脾俞、胃俞),探寻以上患者的热敏穴,取其热敏灸感最强之穴[剔除局部不(微)热远部热、表面不(微)热深部热和非热觉这3种小众灸感]进行疗程为4周的单点灸治,观察比较治疗前后的FD症状积分、尼平指数,探索不同穴位、不同灸感与灸效之间的联系。结果120例FD的患者中脱落4例,治疗后总有效率为70.69%。治疗后患者症状评分优于治疗前(P<0.05),治疗过程中不同热敏灸感的患者在治疗后灸效有统计学意义(P<0.01),其中产生透热灸感患者的灸效明显优于产生传热灸感患者的灸效(P<0.01),其他热敏灸感之间灸效的对比并无差异。不同灸位实施热敏灸疗法之间的灸效总体无统计学意义(P>0.05),其中两两对比仅有在左胃俞穴施灸优于右天枢穴、右胃俞穴施灸产生的灸效(P<0.05)。结论热敏灸治疗FD患者过程中,产生透热灸感患者的灸效明显优于产生传热灸感患者的灸效;不同灸位进行热敏灸对灸效总体无影响,其中两两对比,左胃俞穴施灸灸效优于右天枢穴、右胃俞穴施灸灸效。Objective To observe the clinical efficacy of heat-sensitive moxibustion(single point moxibustion)in the treatment of functional dyspepsia,and to compare its efficacy differences,and to explore the optimized moxibustion treatment(single point moxibustion)scheme of heat-sensitive moxibustion for functional dyspepsia.Methods A total of 120 patients with functional dyspepsia met the criteria,all from outpatient and inpatient departments of our hospital.To find the gold standard——sense positioning method,the high incidence of functional dyspepsia in thermal point area(zhongwan,shu,spleen yu,stomach yu),to explore the above patients of thermal point,take the thermal moxibustion the strongest point(excluding local(micro)thermal far heat,surface not(micro)deep heat and non-thermal feeling the three niche moxibustion)course for single moxibustion treatment,observe before and after treatment of functional dyspepsia symptoms integral,ping index,explore the connection between different acupoints,different moxibustion feeling and moxibustion effect.Results Four cases out of the 120 patients with functional dyspepsia and had a total effective rate of 70.69%.After treatment,patients’symptom score is better than before treatment(P<0.05).After treatment,the effect of moxibustion of patients with different thermal moxibustion feelings varies significantly(P<0.01),among which the moxibustion effect of patients with heat transfer moxibustion feelings is significantly better than that of patients with heat transfer moxibustion feelings(P<0.01),and there is no difference in the comparison between other heat-sensitive moxibustion effects.There is no difference in moxibustion effect between the implementation of thermal moxibustion therapy in different moxibustion locations(P>0.05),among which the comparison of only moxibustion in left stomach Yu point is better than moxibustion in right Weishu point(P<0.05).Conclusions In the process of thermal moxibustion treating patients with functional dyspepsia,the effect of moxibustion pat
