渐进锥度系统ProTaper手用锉冠向下法预备根管治疗的效果

摘　　要：目的观察渐进锥度系统ProTaper手用锉冠向下法预备根管治疗的效果。方法选取2022年1月-2023年6月南京市秦淮区止马营社区卫生服务中心收治的根管治疗患者111例,采用随机数字表法分为观察组(n=56)和对照组(n=55)。观察组实施渐进锥度系统ProTaper手用锉冠向下法预备根管治疗,对照组实施标准锥度系统治疗。比较两组不良反应发生率、治疗有效率、根管壁应力变化及牙周健康水平[探诊深度(PD)、角化龈宽度(KG)、菌斑指数(PI)],并使用简明疼痛量表(BPI)分别测评治疗前、后的疼痛感。结果观察组不良反应总发生率、不同处根管壁应力水平、PI与PD水平、BPI量表评分均低于对照组(P<0.05);观察组治疗总有效率、KG水平高于对照组(P<0.05)。结论实施渐进锥度系统ProTaper手用锉冠向下法预备根管治疗可改善根管壁应力,有效降低不良反应发生率,提升治疗有效率,改善牙周健康和疼痛程度。Objective To observe the application effect of root canal preparation with the crown-down technique using ProTaper hand files of the progressive taper system.Methods A total of 111 patients with root canal treatment admitted to Zhimaying Community Health Service Center in Qinhuai District,Nanjing City from January 2022 to June 2023,were selected and divided into observation group(n=56)and control group(n=55),according to random number table method.The observation group implemented progressive taper system ProTaper,and the control group implemented standard taper system.The incidence of adverse reactions,treatment efficiency,root wall stress changes and periodontal health level[probe(PD),keratinzed gingiva(KG),and plaque index(PI)]were compared between the two groups,and the pain before and after treatment was measured by Brief Pain Inventory(BPI).Results The observation group had a lower total incidence of adverse reactions,reduced levels of root canal wall stress,lower PI and PD levels,and lower BPI scores compared to the control group(P<0.05).Furthermore,the observation group had a higher treatment total efficacy rate and higher KG levels than the control group(P<0.05).Conclusions The implementation of progressive taper system ProTaper Hand root treatment with file crown can improve root wall stress,effectively reduce incidence of adverse reactions,improve treatment efficiency,and improve periodontal health and pain.
