摘 要:目的探讨子宫内膜微量活检联合能量多普勒超声血流参数在早期子宫内膜癌筛查中的应用。方法选取2021年2月—2024年3月在徐州市妇幼保健院需行宫腔镜下诊刮排除子宫内膜病变的患者78例,患者术前均行能量多普勒超声检查及子宫内膜微量取样。随后行宫腔镜下诊断性刮宫。以宫腔镜下诊刮病理为金标准,比较两种取材方式取材满意度的差异;分析子宫内膜微量活检、能量多普勒超声血管化-血流指数对子宫内膜病变的筛查价值。结果子宫内膜微量活检与宫腔镜下诊刮取材满意度分别为89.7%(70/78)、94.9%(74/78),两组结果差异无统计学意义(P>0.05)。子宫内膜微量活检结果、能量多普勒超声血流参数及联合检查筛查子宫内膜疾病的符合率分别为53.8%、81.4%、92.9%。联合检查对子宫内膜癌预测的灵敏度为93.5%,特异度为92.3%,阳性预测值为90.6%,阴性预测值为94.7%。结论子宫内膜微量活检诊断与能量多普勒超声血流参数互为补充,提高筛查的灵敏度。对于子宫内膜疾病的筛查有很好的指导意义。Objective To explore the application of endometrial microbiopsy combined with energy Doppler ultrasound blood flow parameters in early endometrial cancer screening.Methods A total of 78 patients who needed hysteroscopic curettage to rule out endometrial lesions from January 2020 to January 2023 in Xuzhou Maternal and Child Health Hospital were enrolled.The patients underwent energy Doppler ultrasonography and endometrial sampling before surgery.Hysteroscopic diagnostic curettage was performed.Taking the pathology of segmental curettage under hysteroscopy as the gold standard,the difference in satisfaction with the two methods of material collection was compared.To analyze the predictive value of endometrial microbiopsy,energy Doppler ultrasound vascular-blood flow index for endometrial lesions.Results The satisfaction with microbiopsy and hysteroscopy was 89.7%(70/78)and 94.9%(74/78),respectively,there was no significant difference between the two groups(P>0.05).The coincidence rates of endometrial microbiopsy results,energy Doppler ultrasound blood flow parameters and combined examination for the diagnosis of endometrial diseases were 53.8%,81.4%and 92.9%,respectively.The combined test had a sensitivity of 93.5%,a specificity of 92.3%,a positive predictive value of 90.6%and a negative predictive value of 94.7%for the prediction of endometrial cancer.Conclusion Endometrial microbiopsy diagnosis and energy Doppler ultrasound blood flow parameters complement each other to improve the sensitivity of screening.It can be widely used for screening for endometrial diseases.
