米诺环素联合Vitapex糊剂一次性根管治疗根尖周炎伴牙周炎的临床效果及对患者龈沟液炎症因子的影响

摘　　要：目的探讨米诺环素联合Vitapex糊剂一次性根管治疗在根尖周炎伴牙周炎患者中的应用价值。方法选取2023年8月—2024年8月连云港市赣榆区人民医院收治的80例根尖周炎伴牙周炎患者为研究对象,按抛硬币法将其分为对照组和观察组,每组40例。对照组采用氢氧化钙糊剂治疗,观察组采用米诺环素联合Vitapex糊剂治疗,两组患者均持续治疗4周。比较两组的临床疗效、龈沟液炎症因子、牙周指标、不良反应发生情况。结果观察组治疗总有效率为95.00%,高于对照组的80.00%,差异有统计学意义(P<0.05)。治疗后,观察组肿瘤坏死因子-α、C反应蛋白、白细胞介素-17水平均低于对照组,组间差异有统计学意义(P<0.05);观察组龈沟出血指数、菌斑指数评分均低于对照组,牙周袋深度浅于对照组,组间差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论米诺环素联合Vitapex糊剂一次性根管治疗根尖周炎的伴牙周炎效果较好,可降低炎症因子水平,改善牙周指标,且安全性较好。Objective To explore the application value of minocycline combined with one-time root canal treatment using Vitapex paste in apical periodontitis accompanied by periodontitis of patients.Methods Eighty patients with apical periodontitis accompanied by periodontitis,who were admitted to The People's Hospital of Ganyu District,Lianyungang City from August 2023 to August 2024 were selected as the research subjects,they were divided into a control group and an observation group by the coin-tossing method,with 40 patients in each group,the control group was treated with calcium hydroxide paste,while the observation group was treated with minocycline combined with Vitapex paste,both groups of patients were treated continuously for 4 weeks.The clinical efficacy,inflammatory factors in gingival crevicular fluid,periodontal indicators,and occurrence of adverse reactions were compared between the two groups.Results The total effective rate of treatment in the observation group was 95.00%,which was higher than 80.00%in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of tumor necrosis factor-α,C-reactive protein,and interleukin-17 in the observation group were lower than those in the control group,and the differences between the two groups were statistically significant(P<0.05);the gingival crevicular bleeding index and plaque index scores in the observation group were lower than those in the control group,and the depth of the periodontal pocket was shallower than that in the control group,the differences between the two groups were statistically significant(P<0.05).When comparing the incidence of adverse reactions between the two groups,there was no statistically significant difference(P>0.05).Conclusion Minocycline combined with one-time root canal treatment using Vitapex paste shows good efficacy in treating apical periodontitis accompanied by periodontitis.It can reduce the levels of inflammatory factors,improve periodontal indicators,and has good safety.
