前列腺癌穿刺活检组织与根治术后病理组织格利森评分的差异性分析

摘　　要：目的分析经直肠超声引导下的经会阴前列腺癌穿刺组织与前列腺癌根治术病理组织的格利森评分(Gleason score)差异,对比穿刺与根治术病理组织格利森评分相符组患者与不符组患者的临床及超声特征差异。方法回顾性收集行经直肠超声引导下经会阴前列腺穿刺活检诊断为前列腺癌且行前列腺癌根治术的患者。收集患者前列腺穿刺活检组织格利森评分及根治术病理组织格利森评分,采用统计学方法分析二者差异有无统计学意义。收集患者的临床及超声资料,根据穿刺与根治术病理组织格利森评分结果相符或不符对患者进行分组,分析两组患者在临床及超声资料中差异存在统计学意义的特征。结果经直肠超声引导下的经会阴前列腺穿刺组织和根治术病理组织格利森评分差异无统计学意义。穿刺与根治术病理组织格利森评分相符组与不符组患者中差异存在统计学意义的特征为患者病灶形态、病灶部位。结论超声引导下经会阴前列腺癌系统穿刺与前列腺癌根治术后病理格利森评分未存在显著性差异。Objective To analyze the differences in Gleason scores between transrectal ultrasound-guided transperineal prostate biopsy tissues and radical prostatectomy pathological tissues,and to compare the clinical and ultrasound characteristics between patients whose biopsy and radical prostatectomy pathology Gleason scores were concordant versus those that were not.Methods A retrospective collection of patients was conducted.These patients underwent transrectal ultrasound-guided transperineal prostate biopsy and were diagnosed with prostate cancer,followed by radical prostatectomy.The Gleason scores from prostate biopsy tissues and radical prostatectomy pathological tissues were collected and statistically analyzed for differences.Clinical and ultrasound data were obtained,and patients were grouped based on whether their biopsy and radical prostatectomy pathology Gleason scores were concordant.Statistical analysis was then used to identify characteristics differentiating the two groups based on clinical and ultrasound data.Results No significant differences were found in Gleason scores between transrectal ultrasound-guided transperineal prostate biopsy tissues and radical prostatectomy pathological tissues.Statistically significant characteristics distinguishing concordant and non-concordant groups included lesion morphology and lesion location.Conclusions There is no significant difference in Gleason scores between systematic transperineal prostate biopsy under ultrasound guidance and post-radical prostatectomy pathology.
