基于腹腔镜综合教学模式在大外科医师规范化培训中的应用

摘  要:目的探讨基于腹腔镜综合教学模式在大外科医师规范化培训中的应用效果。方法选取2018年9月—2023年7月于莆田学院附属莆田市第一医院大外科接受住院医师规范化培训的外科专业住培学员47名为研究对象。以住培学员个人意愿为基础,将住培学员纳入综合教学组和标准教学组。综合教学组采用基于腹腔镜综合教学模式,标准教学组采用传统的教学模式。课程结束后进行出科考核并发放匿名调查问卷,综合评价教学效果。结果综合教学组的出科考核理论知识、临床操作成绩[(94.2±6.3)分(88.4±7.6)分]高于标准教学组[(78.3±3.2)分、(74.2±3.9)分],差异均有统计学意义(P<0.001);教学效果评价方面,综合教学组在教学内容清晰、课堂氛围积极、团队协作流畅、个人能力得到提升、学习积极性高均优于标准教学组,差异均有统计学意义(P<0.05);综合教学组的各项病例标准操作流程(standard operating procedure,SOP)报告成绩均优于标准教学组,差异均有统计学意义(P<0.001)。结论相比于传统教学方式,基于腹腔镜综合教学模式有效提高了住院医师规范化培训效果,学生的理论出科成绩、操作成绩方面均有所提升,为培养高素质、高技能的腹腔镜外科医师奠定了坚实基础。Objective To explore the application effect of comprehensive teaching mode based on laparoscopy in standardized training of major surgeons.Methods A total of 47 surgical trainees who received standardized training for residents in the department of surgery of the First Hospital of Putian City,Putian University from September 2018 to July 2023 were selected as the research objects.Based on the individual wishes of the trainees,the trainees were included in the comprehensive teaching group and the standard teaching group.The comprehensive teaching group was taught with the comprehensive teaching mode based on laparoscope,and the standard teaching group was taught with the traditional teaching mode.After the course,the course assessment and anonymous questionnaire were distributed to comprehensively evaluate the teaching effect.Results The theoretical knowledge and clinical operation scores of the comprehensive teaching group[(94.2±6.3)points(88.4±7.6)points]were higher than those of the standard teaching group[(78.3±3.2)points,(74.2±3.9)points],and the differences were statistically significant(P<0.001).In terms of teaching effect evaluation,the comprehensive teaching group was better than the standard teaching group in clear teaching content,positive classroom atmosphere,smooth teamwork,improved personal ability and high learning enthusiasm,with statistical significance(P<0.05).The results of standard operating procedure(SOP)reports in the comprehensive teaching group were better than those in the standard teaching group,and the differences were statistically significant(P<0.001).Conclusion Compared with the traditional teaching methods,the comprehensive teaching model based on laparoscopy can effectively improve the effect of standardized training for residents,both the theoretical completion-of-rotation scores and the operational scores of the students have been improved,laying a solid foundation for training high-quality and highly skilled laparoscopic surgeons.
