脑卒中偏瘫患者住院康复期营养不足及营养风险精准筛查与管理效果分析

摘  要:目的:探讨脑卒中偏瘫患者住院康复期营养不足及营养风险精准筛查与管理的效果。方法:选取2021年8月至2022年7月我院收治的100例脑卒中偏瘫患者,随机分为研究组和对照组各50例。研究组患者进行营养风险筛查(NRS2002)并给予营养干预联合康复训练。对照组患者仅进行康复训练。干预共6周。比较两组患者的NRS2002评分、体质指数(BMI)、血清白蛋白、改良Barthel指数、Brunnstrom分级。结果:治疗前,两组患者NRS2002评分、BMI、血清白蛋白水平、改良Barthel指数、Brunnstrom分级比较差异均无统计学意义(P>0.05)。研究组患者治疗1、2、4、6周后的NRS2002评分均低于对照组(P<0.05)。研究组患者治疗4、6周后的BMI均高于对照组(P<0.05)。研究组患者治疗2、4、6周后的血清白蛋白水平、改良Barthel指数均高于对照组(P<0.05)。研究组患者治疗2、4、6周后Brunnstrom分级均优于对照组(P<0.05)。结论:营养风险筛查可评估脑卒中偏瘫患者的营养不足及营养风险,及时给予营养支持,有利于促进康复,提高日常生活活动能力。Objective:To investigate the effects of precise screening and management of nutrient deficiency and nutritional risk in stroke patients with hemiplegia during rehabilitation period.Methods:100 stroke patients with hemiplegia admitted to the hospital from August 2021 to July 2022 were randomly divided into the study group and the control group,with 50 patients in each group.The study group received nutritional risk screening(NRS2002)and nutritional intervention combined with rehabilitation training.The control group received rehabilitation training only.All patients were given a total of 6 weeks of intervention.The two groups were compared in terms of NRS2002 score,body mass index(BMI),serum albumin,modified Barthel index,and Brunnstrom stage.Results:Before treatment,there was no significant difference in NRS2002 score,BMI,serum albumin level,modified Barthel index,Brunnstrom grade between the two groups of patients(P>0.05).After 1,2,4,6 weeks of intervention,NRS2002 scores of the study group were lower than those of the control group(P<0.05).BMI in the study group after 4,6 weeks of intervention was higher than that in the control group(P<0.05).Serum albumin levels and modified Barthel index scores of the study group after 2,4,6 weeks of intervention were higher than those of the control group(P<0.05).After 2,4,6 weeks of intervention,Brunnstrom stage of the study group was better than that of the control group(P<0.05).Conclusion:Nutritional risk screening can help to assess nutrient deficiency and nutritional risk in stroke patients with hemiplegia.Timely nutritional support is beneficial to promote rehabilitation and improve activities of daily living.
