摘  要:约50%的脑卒中幸存者会遗留不同程度的残疾,这严重影响其运动功能和认知功能,而及时有效的认知康复干预对于减缓脑卒中患者认知功能衰退、改善神经可塑性具有重要意义。近年来,随着数字化技术的进步,认知数字疗法(CDT)在脑卒中患者的认知康复中得到了广泛应用,其临床效果日益受到关注。本文通过系统检索文献,全面分析了各种应用形式CDT[虚拟现实(VR)技术、计算机辅助认知康复(CACR)系统、移动健康(mHealth)设备和脑-机接口(BCI)系统]对脑卒中患者认知功能的影响及其优势与局限性,指出各种应用形式的CDT均可改善脑卒中患者的认知功能,但在应用过程中仍面临技术成本、患者接受度、数据安全等挑战,未来亟需进一步开展大样本量的随机对照试验以验证CDT的临床应用效果,并建议将人工智能技术融入CDT系统,从而为脑卒中患者提供更加个性化、精准化的认知康复方案。About 50%of stroke survivors will be left with varying degrees of disability,which seriously affects their motor function and cognitive function.Timely and effective cognitive rehabilitation intervention is of great significance for slowing cognitive function decline and improving neuroplasticity in stroke patients.In recent years,with the development of digital technology,cognitive digital therapy(CDT)has been widely used in the cognitive rehabilitation of stroke patients,and its clinical effect has attracted increasing attention.This paper comprehensively analyzed the effects of various application forms of CDT[virtual reality(VR)technology,computer-assisted cognitive rehabilitation(CACR)system,mobile health(mHealth)devices and braincomputer interface(BCI)system]on the cognitive function of stroke patients and its advantages and limitations through systematic literature search.It is pointed out that CDT in various forms of application can improve the cognitive function of stroke patients,but challenges such as technology cost,patient acceptance and data security are still faced in the application process.In the future,it is urgent to further carry out randomized controlled experiments with a large sample size to verify the clinical application effect of CDT,and it is suggested to integrate artificial intelligence technology into the CDT system,so as to provide more personalized and accurate cognitive rehabilitation programs for stroke patients.
