摘　　要：目的 评估透明角膜双切口设计联合有晶状体眼后房型人工晶状体(ICL)植入术矫正近视合并散光的效果及安全性。方法 回顾性病例观察研究。纳入合肥普瑞眼科医院自2022年3月至2023年5月就诊的近视合并散光行ICL植入术联合透明角膜双切口的患者,综合验光后选取散光范围为-1.0 D~-2.0 D的患者36例(54只眼),在角膜散光陡峭轴处上、下方各做对称的2.8 mm透明角膜双切口。观察患者术前与术后1周、1个月、3个月的裸眼视力(UCVA)、柱镜度、角膜散光变化。结果 36例(54只眼)患者均完成术后3个月的随访,患者术后1周、1个月、3个月裸眼视力较术前均有显著提高,差异均有统计学意义(均P<0.05);术后1周、1个月、3个月角膜散光较术前显著降低(均P <0.05),术后1周、1个月、3个月综合验光散光较术前显著降低(均P <0.05);所有患者均未发生感染、切口渗漏、前房密闭不佳等并发症。结论 透明角膜双切口设计对于-1.0~-2.0D散光的矫正效果显著、稳定性好,手术安全有效,对中度散光的矫正具有可行性。Objective To evaluate the efficacy and safety of double clear corneal incision design combined with implantable collamer lens(ICL)implantation for the correction of myopia combined with astigmatism.Methods Retro-spective case observation study.36 patients(54 eyes)with myopia combined with astigmatism undergoing ICL implanta-tion combined with double clear corneal incision who were seen at Hefei Bright Eye Hospital from March 2022 to May 2023,and patients with astigmatism ranging from-1.0 D to-2.0 D were selected after comprehensive optometry,and sym-metrical 2.8 mm double clear corneal incision.Preoperative and postoperative changes in uncorrected visual acuity(UCVA),column lens degree,and corneal astigmatism were observed at 1 week,1 month,and 3 months.Results 36 pa-tients(54 eyes in total)completed the 3-month postoperative follow-up,and the patients'UCVA at 1 week,1 month,and 3 months after surgery was significantly improved compared to the preoperative period,with statistically significant difer-ences(P<0.05).Corneal astigmatism at 1 week,1 month,and 3 months after surgery was significantly reduced com-pared with the preoperative period(P<0.05),and comprehensive optometric astigmatism at 1 week,1 month,and 3 months after surgery was significantly reduced compared with the preoperative period(P<0.05).No complications such as infection,incision leakage,or poor anterior chamber seal occurred in any of the patients.Conclusions The clear double corneal incision design is effective and stable for the correction of-1.0~-2.0 D astigmatism,and the procedure is safe,ef-fective and feasible for the correction of moderate astigmatism.
