摘 要:目的观察鼻咽癌组织中神经调节蛋白1(NRG1)的表达变化,并分析其与患者预后的关系。方法采用RT-PCR法检测20例新鲜鼻咽癌组织与癌旁组织中的NRG1 mRNA,采用免疫组化法检测83例鼻咽癌组织及癌旁组织中的NRG1蛋白,分析NRG1蛋白表达与鼻咽癌临床病理参数的关系。采用Kaplain-Maier曲线分析比较不同NRG1蛋白表达患者的生存情况,采用Cox回归分析鼻咽癌预后的影响因素。结果鼻咽癌组织中NRG1 mRNA表达及NRG1蛋白高表达率高于癌旁组织(P均<0.05)。NRG1蛋白高表达与鼻咽癌分化程度、T分期、淋巴结转移和临床分期有关(P均<0.05)。NRG1蛋白高表达鼻咽癌患者总生存期(OS)短于NRG1低表达患者(P<0.01)。单因素分析结果显示,NRG1表达(HR=3.23,95%CI 1.62~6.41)、肿瘤分化程度(HR=2.65,95%CI 1.42~4.95)、T分期(HR=2.14,95%CI 1.09~4.21)、临床分期(HR=1.93,95%CI 1.02~3.66)是OS的影响因素,将以上因素纳入多因素分析结果显示,NRG1表达(HR=2.25,95%CI 1.01~4.99)、肿瘤分化程度(HR=2.01,95%CI 1.04~3.89)是OS的独立影响因素。结论鼻咽癌组织中NRG1 mRNA及蛋白表达增高,NRG1蛋白可能参与了鼻咽癌的发生发展,并与鼻咽癌患者的预后有关。Objective To observe the expression changes of neuregulin 1(NRG1)in nasopharyngeal carcinoma(NPC)tissues and to analyze its association with the patients'prognosis.Methods RT-PCR was used to detect NRG1 mRNA levels in 20 fresh NPC tissues and adjacent non-cancerous tissues.Immunohistochemistry(IHC)was performed to assess NRG1 protein expression in 83 NPC tissues and adjacent tissues.The association between NRG1 protein expression and clinicopathological parameters of NPC was analyzed.Kaplan-Meier curves were used to compare survival outcomes be tween patients with high and low NRG1 protein expression.Cox regression analysis was conducted to identify prognostic factors for NPC.Results NRG1 mRNA expression and the high-expression rate of NRG1 protein were significantly high er in NPC tissues than in adjacent tissues(both P<0.05).High NRG1 protein expression was related to tumor differentia tion,T-stage,lymph node metastasis,and clinical stage(all P<0.05).Patients with high NRG1 protein expression had shorter overall survival(OS)than those with low expression(P<0.01).Univariate analysis revealed that NRG1 expression(HR=3.23,95%CI 1.62-6.41),tumor differentiation(HR=2.65,95%CI 1.42-4.95),T-stage(HR=2.14,95%CI 1.09-4.21),and clinical stage(HR=1.93,95%CI 1.02-3.66)were significant influencing factors for OS.Multivariate analysis identified NRG1 expression(HR=2.25,95%CI 1.01-4.99)and tumor differentiation(HR=2.01,95%CI 1.04-3.89)were the independent prognostic factors for OS.Conclusion The expression of NRG1 mRNA and protein in NPC tissues increases,and NRG1 protein may be involved in the development and progression of NPC and is related to the prognosis of NPC patients.
