摘　　要：目的:分析腹腔镜下胆总管切开术联合胆道镜下取石术治疗胆囊结石合并胆总管结石的临床效果。方法:以昆山市第五人民医院(2021年1月—2024年6月)收治的62例胆囊结石合并胆总管结石患者为研究对象,按随机数字表法分组,每组31例。观察组行腹腔镜下胆总管切开术联合胆道镜下取石术,对照组行小切口开腹手术。对比两组治疗效果。结果:观察组手术时间、住院时间短于对照组,术中出血量少于对照组(P<0.05)。观察组并发症总发生率低于对照组(P<0.05)。术后7d,两组C反应蛋白、白细胞介素-6、肿瘤坏死因子-α水平下降,且与对照组比较,观察组更低(P<0.05)。结论:腹腔镜下胆总管切开术联合胆道镜下取石术治疗胆囊结石合并胆总管结石,可改善手术指标,减轻炎性反应,预防并发症。Objective:To analyze the clinical effect of laparoscopic choledochotomy combined with choledochoscopic lithotomy in treatment of gallbladder stones with common bile duct stones.Methods:This study encompassed a cohort of 62 patients diagnosed with both gallbladder stones and common bile duct stones who were admitted to Kunshan Fifth People's Hospital(between January 2021 and June 2024).They were divided into two groups using a random number table method,with 31 cases in each group.The observation group underwent laparoscopic choledochotomy and choledochoscopic lithotomy;the control group underwent small incision laparotomy.The treatment effects were compared between the two groups.Results:The operation time and hospitalization duration were significantly shorter for the observation group compared to the control group(P<0.05).Additionally,the intraoperative blood loss was also less in the observation group than in the control group(P<0.05).The control group had a higher overall rate of complications compared to the observation group(P<0.05).Conclusion:Laparoscopic choledochotomy combined with choledochoscopic lithotomy for the treatment of gallbladder stones with common bile duct stones can improve surgical indicators,reduce inflammation and prevent complications.
