基于“气机升降理论”探讨视神经萎缩的病因病机及治疗

摘　　要：“气机升降理论”为中医理论的重要组成,气机的升降出入是生命活动的重要表现。目为九窍之一,居于上,有赖于精微物质的滋养,因此,“气机升降理论”在治疗眼科疾病中发挥重要作用。视神经萎缩是不可逆性致盲性眼病,病机复杂,是现代医学难以治疗的疾病。中医学认为,视神经萎缩的发生多与情志抑郁、气血不足有关。本文基于气机升降理论,以中医理论为指导,从脉道不畅和清阳不升2个方面,探讨其病因病机及中医治疗。通过对病因病机的分析,以调整脏腑气机为指导,运用中药内服配合针刺进行治疗,效果明显,并以病案说明笔者在临床运用“气机升降理论”治疗视神经萎缩的体会,为临床治疗提供新思路。The theory of Qi ascending and descending is a crucial component of Traditional Chinese Medicine(TCM),as the dynamic movement of Qi(ascending,descending,entering,and exiting)represents a vital manifestation of life activities.The eyes,one of the nine orifices,are located in the upper body and rely on the nourishment of essential substances.Thus,the theory of Qi movement and regulation plays a significant role in treating ophthalmic diseases.Optic atrophy,an irreversible blinding eye disease with complex pathogenesis,remains challenging to address in modern medicine.TCM posits that its occurrence is closely related to emotional depression and deficiency of Qi and blood.Guided by the theory of Qi ascending and descending,this paper explores the etiology and pathogenesis of optic atrophy from two perspectives:Impeded meridians and failure of clear Yang to ascend,while proposing TCM-based therapeutic approaches.Through analyzing the disease mechanisms,Chinese medicine combined with acupuncture was utilized to regulate visceral Qi movement,achieving notable clinical efficacy.A case study is presented to illustrate the application of this theory in treating optic atrophy,offering novel insights for clinical practice.
