摘 要:恶性肿瘤已成为威胁人类健康的主要疾病之一。放射治疗和药物化疗在肿瘤综合治疗中发挥着重要作用,然而其在杀伤肿瘤细胞的同时,也不可避免地对心脏产生一定的毒副作用,可引起癌症治疗相关心功能障碍(Cancer Therapeutics-Related Cardiac Dysfunction,CTRCD)。及早发现和准确评估CTRCD,对于指导癌症治疗方案的优化调整,改善患者预后至关重要。超声心动图、心脏磁共振、核素显像等多模态影像技术,以其无创、动态、定量等特点,为CTRCD的检测和评估提供了不可或缺的手段。本文综述了近年来多模态影像医学在CTRCD诊疗中的研究进展,重点介绍了超声心动图新技术(如斑点追踪成像、三维超声、左室压力-应变分析)、心脏磁共振成像新方法(如T1 mapping、应变分析)以及核素显像新进展(如PET代谢与灌注显像)的基本原理和临床应用价值,并展望了多模态影像在CTRCD早期诊断和疗效评估中的整合应用前景。Malignant tumor has become one of the main diseases threatening human health.Radiotherapy and drug chemotherapy play an important role in the comprehensive treatment of cancer.However,while killing tumor cells,they also inevitably have certain toxic and side effects on the heart,which can cause cancer therapeutics related cardiac dysfunction(CTRCD).Early detection and accurate evaluation of CTRCD is very important for guiding the optimization and adjustment of cancer treatment plan and improving the prognosis of patients.Mul-timodal imaging techniques such as echocardiography,cardiac magnetic resonance imaging and radionuclide imaging,with their non-invasive,dynamic and quantitative characteristics,provide an indispensable means for the detection and evaluation of CTRCD.This paper reviews the research progress of multimodal imaging medicine in the diagnosis and treatment of CTRCD in recent years,focusing on the basic principles and clinical application value of new echocardiography technologies(such as speckle tracking imaging,three-dimensional ultrasound,left ventricular pressure strain analysis),new cardiac magnetic resonance imaging methods(such as T1 mapping,strain analysis)and new progress of radionuclide imaging(such as pet metabolism and perfusion imaging),and looks forward to the integrated application prospect of multimodal imaging in the early diagnosis and curative effect evaluation of CTRCD.
