摘　　要：目的评价云南玉溪市DOACs的可及性。方法采用回顾性分析方法,选取玉溪市医疗机构DOACs的相关数据进行分析,对5种DOACs进行可及性评价。对比各药品单剂量价格差异;采用WHO/HAI可获得率分析方法进行分层调查;采用WHO/HAI标准调查法、药品致贫作用评价法和灾难性药品支出评价法评价DOACs的可负担性。结果调查机构包括共60家医疗机构。在可获得性和可负担性方面,VKA优于其他DOACs。综合评价结果,VKA的可及性要优于DOACs;而DOACs中,利伐沙班可及性要优于其他药品。结论目前DOACs应用已十分广泛,对需长期使用患者有一定的经济负担,药品的可及性将会大大影响患者的依从性。目前本地区居民的DOACs的可及性仍不高,还需从多方面入手,改善其可及性。Objective To evaluate the accessibility of DOACs in Yuxi,Yunnan Province.Methods The relevant data of DOACs in Yuxi medical institutions were selected for analysis,and the accessibility of five kinds of DOACs was evaluated.The price difference of single dose of each drug was compared;The WHO/HAI availability rate analysis method was used for stratified the survey;The affordability of DOACs was evaluated by WHO/HAI standard survey method,the evaluation method of drug poverty-causing effects and the evaluation method of catastrophic drug expenditure.Results The surveyed organizations included a total of 60 medical organizations.VKA was superior to other DOACs in terms of availability and affordability.The comprehensive evaluation results showed that the accessibility of VKA was better than that of DOACs;While in DOACs,the accessibility of rivaroxaban was better than that of other drugs.Conclusions At present,DOACs has been widely used,which has a certain economic burden on patients who need to use it for a long time,and the availability of drugs will greatly affect patients compliance.The current accessibility of DOACs for the population in this region is still not high,and there is a need to work on various aspects to improve its accessibility.
