下颌阻生智齿拔除患者牙科焦虑症的调查分析

摘　　要：目的了解下颌阻生智齿拔除患者牙科焦虑症的发生现状及相关因素。方法通过患者一般资料调查表和改良牙科焦虑量表对患者牙科焦虑症的发生现状和相关因素进行调查。结果下颌阻生智齿拔除术患者牙科焦虑症的发病率为24.7%。性别、年龄、学历、职业、疼痛耐受情况、平时心理素质、对拔牙知识了解程度、本次拔牙是否需亲友陪伴、有无不愉快的牙科就诊经历、拔牙时医护人员的态度、周围患者焦虑状态、对诊室周围环境设备满意度等是牙科焦虑症发生的相关因素,差异有统计学意义(P<0.05)。以MDAS评分作为因变量,拟合多重线性回归模型,结果显示,女性、疼痛的耐受越差、平时心理素质越差的患者MDAS评分较高。结论下颌阻生智齿拔除术患者牙科焦虑症的发病率较高,相关因素较多,女性、疼痛耐受差、平时心理素质差是牙科焦虑症发生的独立高风险因素。Objective To investigate the current situation and related factors of dental anxiety in patients with mandibular impacted wisdom tooth extraction.Methods The general data questionnaire and the modified dental anxiety scale were used to investigate the current situation and related factors of dental anxiety.Results The incidence of dental anxiety in mandibular impacted wisdom tooth extraction patients was 24.7%.Among the factors related to the occurrence of dental anxiety disorder,there were statistically significant differences in gender,age,education,occupation,pain tolerance,usual psychological quality,understanding of tooth extraction knowledge,whether the tooth extraction needed to be accompanied by friends and relatives,pleasant dental experience,attitude of medical staff during tooth extraction,anxiety state of surrounding patients,and satisfaction with the surrounding environment and equipment in the office(P<0.05).MDAS score was used as the dependent variable to fit the multiple linear regression model,and the results showed that the MDAS score was higher in women,patients with worse pain tolerance and psychological quality.Conclusions The incidence of dental anxiety is higher in mandibular impacted wisdom tooth extraction patients,and there are many related factors.Female,poor pain tolerance and poor psychological quality are independent high risk factors for dental anxiety.
