良性特发性眼睑痉挛与干眼症的相关性研究

摘　　要：目的研究良性特发性眼睑痉挛与干眼症的相关性。方法将2021年5月~2022年10月就诊于成都市龙泉驿区中医医院眼科门诊诊断为良性特发性眼睑痉挛(benign essential blepharospasm,BEB)患者作为研究对象。观察组为BEB患者共50例;对照组为BEB患者配偶共30例,男女比例与观察组匹配,排除患有全身性基础疾病及眼部疾病。分别对研究对象进行干眼症相关检查包括眼表疾病指数(OSDI)、泪膜破裂时间(TBUT)、泪液分泌试验(Schirmer-I)。结果观察组OSDI评分中位数为31.25(22.17,47.75),对照组OSDI评分中位数为15.00(12.50,47.00),两组OSDI评分存在统计差异(z=-5.087,P<0.001);观察组泪膜破裂时间为5.58±1.72 s,对照组泪膜破裂时间为9.03±2.22 s,两组泪膜破裂时间均数存在着统计差异(t=-7.767,P<0.001);观察组泪液分泌中位数为8.00(7.00,10.00)mm,对照组泪液分泌中位数为10.00(8.00,12.00)mm,两组泪液分泌试验存在统计差异(z=-2.394,P<0.05)。结论特发性眼睑痉挛与干眼症相互影响,应进一步探索二者的发病机制及综合性治疗策略。Objective To study the correlation between benign idiopathic blepharospasm and dry eye.Methods From May 2021 to October 2022,the patients diagnosed as benign essential blepharospasm(BEB)in the outpatient department of The Traditional Chinese Medicine Hospital of Longquanyi,Chengdu were taken as the research objects.The observation group consisted of 50 patients with BEB.The control group consisted of 30 spouses of BEB patients,and the ratio of male to female matched that of the observation group,excluding patients with systemic basic diseases and eye diseases.The indexes related to dry eye were examined,including ocular surface disease index(OSDI),tear film rupture time(TBUT)and tear secretion test(Schirmer-I).Results The median OSDI score of the observation group was 31.25(22.17,47.75),while that of the control group was 15.00(12.50,47.00),showing a statistical difference between the two groups(z=-5.087,P<0.001).The tear film rupture time was 5.58±1.72 s in the observation group and 9.03±2.22 s in the control group,and there was a statistical difference in the mean tear film rupture time between the two groups(t=-7.767,P<0.001).The median tear secretion of the observation group was 8.00(7.00,10.00)mm,while that of the control group was 10.00(8.00,12.00)mm.There was a statistical difference between the two groups in tear secretion test(z=-2.394,P<0.05).Conclusion Benign essential blepharospasm and dry eye interact with each other,so we should actively explore their pathogenesis and combined treatment strategy.
