SMILE与FS-LASIK术后24小时内视力恢复与自觉症状的比较

摘　　要：目的比较飞秒激光小切口角膜基质透镜取出术(SMILE)和飞秒激光制瓣准分子激光原位角膜磨镶术(FS-LASIK)术后24h内裸眼视力与自觉症状,探讨两种手术方式术后极早期恢复的差异。方法根据纳排标准,纳入2022年11月01日到2022年12月31日于成都中医大银海眼科医院屈光手术科行SMILE或FS-LASIK手术矫正屈光不正的患者89例160眼,根据手术方式将其分为SMILE组(57例101眼)、FS-LASIK组(32例59眼)。分别比较两组患者术后1 h、2 h、4 h、24h时裸眼视力与自觉症状评分。采用SPSS 26.0软件对所收集的数据进行分析处理。结果术后1 h到术后24 h,两组患者裸眼视力稳步提升,且在术后1 h、2 h、4 h时FS-LASIK组裸眼视力恢复优于SMILE组(P<0.05),术后24 h时两组患者裸眼视力无明显差异;比较两组患者自觉症状积分,术后2 h、4 h、24 h时FS-LASIK组患者术眼自觉症状总积分明显高于SMILE组(P<0.05);术后24 h内所有时间点FS-LASIK组疼痛评分更高;两组患者术后24 h内各时间点术眼干涩感评分无统计学差异;两组患者术后2 h、4 h时术眼畏光及流泪评分有统计学差异(P<0.05),FS-LASIK组术眼畏光、流泪症状均更为明显。结论术后4 h之内,行FS-LASIK患者UCVA恢复优于行SMILE者,术后24 h时两组UCVA未见明显差异;但术后24 h内SMILE组总体自觉症状较FS-LASIK更轻。Objective To compare the uncorrected visual acuity(UCVA)and self-reported symptoms within 24 hours after Small Incision Lenticule Extraction(SMILE)and Femtosecond Laser-Assisted In Situ Keratomileusis(FS-LASIK),aiming to investigate the differences in the very early recovery phase following the two surgical procedures.Methods In accordance with the inclusion and exclusion criteria,a total of 89 patients(160 eyes)who underwent SMILE or FS-LASIK for myopia correction at the Refractive Surgery Department of INEYE Hospital Affiliated to Chengdu University of Chinese Medicine,from November 1,2022,to December 31,2022,were enrolled.Patients were allocated to the SMILE group(57 cases,101 eyes)or the FS-LASIK group(32 cases,59 eyes)based on the surgical method.UCVA and self-reported symptom scores were compared between the two groups at 1 hour,2 hours,4 hours,and 24 hours postoperatively.Collected data were analyzed using SPSS 26.0 software.Results From 1 hour to 24 hours postoperatively,UCVA improved steadily in both groups,with the FS-LASIK group demonstrating superior recovery at 1 hour,2 hours,and 4 hours(P<0.05).At 24 hours postoperatively,no significant difference in UCVA was observed between the two groups.Comparison of self-reported symptom scores revealed significantly higher total symptom scores in the FS-LASIK group at 2 hours,4 hours,and 24 hours postoperatively(P<0.05).Pain scores were consistently higher in the FS-LASIK group at all time points within 24 hours.No statistical difference was found in dryness scores between the two groups at any time point within 24 hours.Photophobia and tearing scores were statistically different at 2 hours and 4 hours postoperatively(P<0.05),with the FS-LASIK group experiencing more pronounced symptoms.Conclusion Within 4 hours postoperatively,UCVA recovery was superior in patients undergoing FS-LASIK compared to SMILE.However,at 24 hours postoperatively,no significant difference in UCVA was observed between the two groups.Nonetheless,the SMILE group reported less severe over
