双侧腮腺多发基底细胞腺癌1例

摘　　要：目的探讨基底细胞腺癌的诊断、鉴别诊断、治疗、预后,揭示基底细胞腺癌多发的可能性,同时为双侧腮腺肿物发病的类型提供临床数据的支持。方法排除手术禁忌后,对双侧腮腺基底细胞腺癌的患者进行手术,并对切除的肿物进行病理分析、查阅回顾文献,得出结果及结论。结果本病例基底细胞腺癌在临床表现上与多形性腺瘤、基底细胞腺瘤无明确差异,在病理学表现上与基底细胞腺样瘤、腺样囊性癌、多形性腺瘤、上皮性肌上皮癌存在相似之处。结论基底细胞腺癌与良性肿瘤在临床表现、影像学表现多无明确差异,大多需术后病理明确诊断。Objective To discuss the diagnosis,differential diagnosis,treatment and prognosis of basal cell adenocarcinomas by describing the clinical,imaging and pathological manifestations of a patient and literature review,and to provide clinical data for classifying the onset types of bilateral parotid gland masses.Methods A patient with bilateral basal cell adenocarcinomas of the parotid gland was operated after ruling out the surgical contraindications.The resected mass was pathologically examined.Through literature review,the clinical diagnosis and treatment were summarized.Results In this case,the basal cell adenocarcinoma had no definite differences in clinical manifestations from pleomorphic adenoma and basal cell adenoma.Its pathological manifestation was similar to that of basal cell adenoma,adenoid cystic carcinoma,pleomorphic adenoma and epithelial myoepithelial carcinoma.Conclusion Basal cell adenocarcinoma of the parotid gland and benign tumors have no clear difference in clinical and imaging manifestations,and most of them need to be diagnosed by postoperative pathology.
