摘 要:目的探讨围绝经期干眼症患者结膜上皮细胞微小RNA(miR)-590、CC趋化因子配体20(CCL20)水平及临床意义。方法选取2021年5月至2023年11月于哈尔滨爱尔眼科医院就诊的73例(146只眼)围绝经期干眼症患者作为研究组,另选取同期哈尔滨爱尔眼科医院61例无干眼症围绝经期健康体检女性作为对照组。收集2组临床资料。采用实时荧光定量聚合酶链反应检测2组结膜上皮细胞miR-590水平,采用酶联免疫吸附试验检测2组结膜上皮细胞CCL20水平并进行比较。比较不同程度干眼症患者结膜上皮细胞miR-590、CCL20水平。采用受试者工作特征(ROC)曲线分析结膜上皮细胞miR-590、CCL20对围绝经期女性发生干眼症的诊断价值。采用多因素Logistic回归分析围绝经期女性发生干眼症的影响因素。采用Pearson相关分析干眼症患者白细胞介素(IL)-33、IL-6、肿瘤坏死因子-α(TNF-α)水平与miR-590、CCL20水平的相关性。结果研究组和对照组年龄、体质量指数、患病时间,以及血清促卵泡生成素、促黄体生成素、雌二醇水平比较,差异均无统计学意义(P>0.05);研究组IL-33、IL-6、TNF-α、CCL20水平均明显高于对照组,miR-590水平低于对照组,差异均有统计学意义(P<0.05)。随着干眼症严重程度增加,miR-590水平逐渐降低,CCL20水平逐渐升高。ROC曲线分析结果显示,miR-590、CCL20单独及二者联合检测诊断围绝经期女性发生干眼症的AUC分别为0.889、0.899、0.949,二者联合检测诊断围绝经期女性发生干眼症的AUC大于miR-590、CCL20单独检测的AUC,差异均有统计学意义(Z=2.053、2.306,P=0.040、0.021)。多因素Logistic回归分析结果显示,IL-33、IL-6、TNF-α、CCL20水平升高均为围绝经期女性发生干眼症的危险因素(P<0.05);miR-590水平升高为围绝经期女性发生干眼症的保护因素(P<0.05)。Pearson相关分析结果显示,干眼症患者miR-590水平与IL-33、IL-6、TNF-α水平均呈负�Objective To investigate the expression and clinical significance of microRNA(miR)-590 and CC chemokine ligand 20(CCL20)in conjunctival epithelial cells of perimenopausal dry eye patients.Methods A total of 73 patients(146 eyes)with perimenopausal dry eye treated in Harbin Aier Eye Hospital from May 2021 to November 2023 were selected as the study group,and 61 healthy perimenopausal women without dry eye were selected as the control group during the same period.The clinical data of the two groups were collected.The level of miR-590 in conjunctival epithelial cells of the two groups was detected by real-time fluorescent quantitative polymerase chain reaction,and the level of CCL20 in conjunctival epithelial cells was detected by enzyme-linked immunosorbent assay of the two groups.The levels of miR-590 and CCL20 in conjunctival epithelial cells of patients with different degrees of dry eye were compared.The receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of miR-590 and CCL20 in conjunctival epithelial cells for dry eye in perimenopausal women.Multivariate Logistic regression was used to analyze the influencing factors of dry eye in perimenopausal women.Pearson correlation analysis was used to analyze the correlation between interleukin(IL)-33,IL-6,tumor necrosis factor-α(TNF-α)levels and miR-590,CCL20 levels in dry eye patients.Results There was no significant difference in age,body mass index,disease time,and serum follicle-stimulating hormone,luteinizing hormone,and estradiol levels between the study group and the control group(P>0.05).The levels of IL-33,IL-6,TNF-αand CCL20 in the study group were significantly higher than those in the control group,and the level of miR-590 was lower than that in the control group,and the differences were statistically significant(P<0.05).With the increase of the severity of dry eye,the level of miR-590 was gradually decreased,and the level of CCL20 was gradually increased.ROC curve analysis showed that the AUC of miR-590,CCL20 and their c
