颈动脉粥样硬化性重度狭窄患者血管超声评估参数与缺血性脑卒中的相关性探讨

摘　　要：探讨颈动脉粥样硬化引发重度狭窄的情况下,血管超声检测参数与同侧缺血性脑卒中发病之间的相关性。对2023年1月—2024年12月在川北医学院附属医院接受治疗的132名颈动脉粥样硬化重度狭窄患者进行研究,根据近6个月内是否发生过同侧缺血性脑卒中分为脑卒中组和对照组,探讨血管超声参数与缺血性脑卒中的关系。结果显示,脑卒中组的狭窄段收缩期峰值流速(peak systolic velocity in the stenosis section,PSVs)、狭窄段舒张期末流速(end diastolic velocity in the stenosis section,EDVs)和收缩期峰值流速的比值(PSVs/PSVd)显著高于对照组,PSVd显著低于对照组。多因素Logistic回归分析结果显示,溃疡斑块是同侧缺血性脑卒中的独立危险因素。超声诊断溃疡斑块的敏感度和特异度分别为82.35%、93.94%,与CTA具有较高一致性。研究发现,颈动脉重度狭窄患者中,溃疡斑块是缺血性脑卒中的独立危险因素。血管超声在诊断溃疡斑块方面具有高准确性,可作为有效评估工具。To explore the correlation between vascular ultrasound parameters and ipsilateral ischemic stroke in the case of severe stenosis caused by carotid atherosclerosis.132 patients with severe carotid atherosclerosis stenosis who were treated in the Affiliated Hospital of North Sichuan Medical College from January 2023 to December 2024 were studied.They were divided into stroke group and control group according to whether there had been ipsilateral ischemic stroke in the past 6 months.The relationship between vascular ultrasound parameters and ischemic stroke was discussed.The results showed that the peak systolic velocity in the stenosis section(PSVs),end diastolic velocity in the stenosis section(EDVs),and the ratio of peak systolic velocity to peak systolic velocity(PSVs/PSVd)in the stroke group were significantly higher than those in the control group,while the PSVd was signifi cantly lower than that in the control group.The results of multiple logistic regression analysis showed that ulcerative plaques are an independent risk factor for ipsilateral ischemic stroke.The sensitivity and specifi city of ultrasound diagnosis of ulcer plaques were 82.35%and 93.94%,respectively,which showed high consistency with CTA.Research has found that ulcerative plaques are an independent risk factor for ischemic stroke in patients with severe carotid artery stenosis.Vascular ultrasound has high accuracy in diagnosing ulcer plaques and can be used as an eff ective evaluation tool.
