摘  要:目的探讨计算机辅助设计/计算机辅助制作(CAD/CAM)技术在可摘局部义齿支架制备中的应用效果。方法选取2021年1月至2023年1月于医院行义齿修复治疗的60例牙列缺损患者作为研究对象,随机分为对照组和试验组,每组30例。两组均采用可摘局部义齿修复治疗。对照组采用失蜡铸造方式制备义齿支架,试验组采用CAD/CAM技术辅助制备义齿支架。两组均随访3个月。比较两组义齿就位率、基托密合度,治疗前后的咀嚼效率、探诊深度,以及患者对义齿支架舒适度、美观度、异味度的满意度和随访期间的牙周并发症(继发龋齿、牙周病、义齿性口炎)发生率。结果试验组义齿就位率、基托密合率均高于对照组,差异有统计学意义(P<0.05)。治疗前,两组咀嚼效率、探诊深度比较,差异无统计学意义(P>0.05);治疗后,两组咀嚼效率均高于同组治疗前,探诊深度均低于同组治疗前,且试验组咀嚼效率高于对照组,探诊深度低于对照组,差异有统计学意义(P<0.05)。试验组对义齿支架舒适度、美观度、异味度的满意度均高于对照组,随访期间牙周并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论采用CAD/CAM技术辅助制备可摘局部义齿支架,可提高支架就位率、基托密合度、患者的咀嚼效率及满意度,降低探诊深度和牙周并发症发生率。Objective To explore the application effect of computer-aided design and computer-aided manufacture(CAD/CAM)technology in the fabrication of removable partial denture(RPD)frameworks.Methods With the selection of sixty patients with dentition defect who underwent denture restoration treatment in hospitals from January 2021 to January 2023 as the study subjects,they were randomly divided into the control group and the experimental group,with 30 cases in each group.Removable partial denture restoration treatment was used in both groups.The lost wax casting method was used to prepare denture supports in the control group,and CAD/CAM technology was used to assist in the fabrication of denture frameworks in the experimental group.Both groups were followed up for 3 months.The two groups were compared in terms of denture placement rate,base fit,masticatory efficiency,probing depth before and after treatment,patients'satisfaction with the comfort,aesthetics,odor of the denture frameworks,and the incidence of periodontal adverse reactions(secondary caries,periodontal disease,denture stomatitis)during follow-up.Results The denture placement rate and base fit rate of the experimental group were higher than those of the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no statistically significant differences in masticatory efficiency and probing depth between the two groups(P>0.05);After treatment,the masticatory efficiency in both groups was higher than before,and the probing depth was lower than before,and the masticatory efficiency in experimental group was higher than that in control group,and the probing depth was lower than that in control group,with statistical significance(P<0.05).The satisfaction of the experimental group on the comfort,aesthetics and odor of the denture framework was higher than that of the control group,and the incidence of periodontal adverse reactions during follow-up was lower than that of the control group,with statistical significance(P<0.05)
