干眼症患者睡眠质量与眼表状态、泪液分泌量、泪膜稳定性的相关性研究

摘　　要：目的:分析干眼症患者睡眠质量与眼表状态、泪液分泌量、泪膜稳定性的相关性。方法:回顾性分析2023年1月至6月福建厦门科宏眼科医院眼科收治的干眼症患者96例作为研究对象,根据匹兹堡睡眠指数(PSQI)评分分为轻度睡眠障碍组(n=43)和中重度睡眠障碍组(n=53)。以眼表疾病指数量表(OSDI)评估眼表状态,以基础泪液分泌试验Ⅰ试验(SⅠT)评估泪液分泌量,以泪膜破裂时间(BUT)评定泪膜稳定性,比较2组的一般资料以及OSDI、SⅠT、BUT水平,并分析上述指标与干眼症患者睡眠质量的相关性。结果:轻度睡眠障碍组OSDI评分低于中重度睡眠障碍组,SⅠT值、BUT值高于中重度睡眠障碍组(P<0.05);相关性分析显示,干眼症患者PSQI评分与OSDI评分正相关关系,与SⅠT、BUT值评分负相关(P<0.05)。结论:干眼症患者眼表状态、泪液分泌量、泪膜稳定性与其睡眠质量存在相关性,眼表状态越差、泪液分泌量以及泪膜稳定性越低其睡眠质量越差。Objective:To analyze the correlation between sleep quality and ocular surface condition,tear secretion,and tear film stability in patients with dry eye disease(DED).Methods:A retrospective analysis was conducted on a total of 96 patients with DED treated at the Department of Ophthalmology,Xiamen Kehong Eye Hospital,from January to June 2023.Based on Pittsburgh Sleep Quality Index(PSQI)scores,patients were divided into a mild sleep disorder group(n=43)and a moderate-to-severe sleep disorder group(n=53).The Ocular Surface Disease Index(OSDI)was used to assess ocular surface condition,the SchirmerⅠTest(SⅠT)was employed to evaluate tear secretion,and tear film break-up time(BUT)was used to assess tear film stability.General characteristics,OSDI,SⅠT,and BUT levels were compared between the two groups,and the correlations between these indicators and sleep quality in DED patients were analyzed.Results:The OSDI score was significantly lower in the mild sleep disorder group compared to the moderate-to-severe sleep disorder group,while SⅠT and BUT values were significantly higher(P<0.05).Correlation analysis showed that PSQI scores were positively correlated with OSDI scores and negatively correlated with SⅠT and BUT scores in DED patients(P<0.05).Conclusion:Sleep quality in DED patients is correlated with ocular surface condition,tear secretion,and tear film stability.Poorer ocular surface condition,lower tear secretion,and reduced tear film stability are associated with worse sleep quality.
