摘 要:目的 探讨64排CT后处理技术诊断原发性肝癌(PHC)的临床价值。方法 选取2021年7月—2023年10月冠县新华医院收治的80例疑似PHC患者,均行64排CT检查和术后病理检查,比较64排CT后处理技术诊断PHC不同临床分期影像学表现和64排CT后处理技术与术后病理检查诊断PHC情况,分析64排CT后处理技术与术后病理检查诊断PHC临床分期符合情况。结果 64排CT后处理技术诊断PHC T1或T2期患者各影像学表现占比均低于T_3或T_4期患者,差异有统计学意义(P<0.05)。80例疑似PHC患者中,经术后病理检查确诊PHC患者71例(88.75%),64排CT后处理技术诊断PHC患者72例(90.00%)。64排CT后处理技术诊断PHC的敏感度为98.59%,特异度为77.78%,准确率为96.25%,阳性预测值为97.22%,阴性预测值为87.50%。64排CT后处理技术与术后病理检查诊断PHC的一致性良好(Kappa=0.803,P <0.05)。64排CT后处理技术与术后病理检查诊断PHC临床分期总符合率为91.55%。结论 64排CT后处理技术诊断PHC及其临床分期准确率较高,与术后病理检查诊断一致性好。Objective To investigate the clinical value of 64-slice CT post-processing technique in the diagnosis of primary hepatocellular carcinoma(PHC).Methods A total of 80 patients with suspected PHC admitted to Guanxian Xinhua Hospital from July 2021 to October 2023 were selected.All patients underwent 64-slice CT examination and postoperative pathological examination.The imaging findings of 64-slice CT post-processing technology in the diagnosis of PHC in different clinical stages and the diagnosis of PHC by 64-slice CT post-processing technology and postoperative pathological examination were compared,and the coincidence of 64-slice CT postprocessing technology and postoperative pathological examination in the diagnosis of PHC clinical stage was analyzed.Results The proportion of imaging findings in patients with PHC T1 or T2 diagnosed by 64-slice CT post-processing technique was lower than that in patients with T3 or T4,and the difference was statistically significant(P<0.05).Among the 80 patients with suspected PHC,71 patients(88.75%)were diagnosed with PHC by postoperative pathological examination,and 72 patients(90.00%)were diagnosed with PHC by 64-slice CT post-processing technique.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of 64-slice CT post-processing technique in the diagnosis of PHC were 98.59%,77.78%,96.25%,97.22%and 87.50%,respectively.The consistency between 64-slice CT post-processing technique and postoperative pathological examination in the diagnosis of PHC was good(Kappa=0.803,P<0.05).The total coincidence rate of 64-slice CT post-processing technique and postoperative pathological examination in the diagnosis of PHC clinical stage was 91.55%.Conclusions The accuracy of 64-slice CT post-processing technique in the diagnosis of PHC and its clinical staging is high,and the consistency with postoperative pathological examination is good.
