子宫内膜癌中医证型与临床病理特征相关性研究

摘　　要：目的探究子宫内膜癌临床病理特征与中医证型的相关性。方法选取2021年4月-2023年4月于我院进行治疗的70例子宫内膜癌患者作为研究对象,根据患者的症状判断中医证型(湿热下注型、肾阴虚型、脾虚肝郁型和湿毒淤结型),同时统计患者的临床病理特征,通过Spearman相关性分析临床病理特征与中医证型的相关性。结果70例子宫内膜癌患者中医证型为湿热下注型、肾阴虚型、脾虚肝郁型和湿毒淤结型的比例分别为21.43%、44.29%、15.71%和18.57%,其中肌层浸润深度<1/2和≥1/2的患者比例分别为64.29%和35.71%;组织学分级为G1、G2、G3的患者比例分别为68.57%、17.14%和14.29%;分期为Ⅰ期、Ⅱ期、Ⅲ期的患者比例分别为70.00%、22.86%和7.14%;病理类型为内膜样腺癌和非内膜样腺癌的患者比例分别为90.00%和10.00%。Spearman结果表明,肌层浸润深度与湿热下注型(r=0.346,P<0.001)、肾阴虚型(r=0.582,P<0.001)、脾虚肝郁型(r=0.927,P<0.001)和湿毒淤结型(r=0.559,P<0.001)之间均存在显著的正相关关系(P<0.05),而病理类型则与四种中医证型之间不存在相关性(P>0.05)。结论子宫内膜癌中医证型与肌层浸润深度、组织学分级和分期相关,可将中医证型作为评价患者预后的指标,同时更好地发挥中医药治疗子宫内膜癌的作用。Objective To explore the correlation between clinicopathological features of endometrial carcinoma and TCM syndrome types.Methods Seventy cases of endometrial cancer patients treated in our hospital from April 2021 to April 2023 were selected as research objects.TCM syndrome types(dampness-heat type,kidney-yin deficiency type,spleen deficiency liver-depression type and dampness-toxin congestion type)were determined according to the symptoms of the patients.Meanwhile,clinicpathological characteristics of the patients were analyzed.Spearman correlation was used to analyze the correlation between clinicopathological features and TCM syndrome types.Results The proportion of Traditional Chinese Medicine syndrome types for 70 patients with endometrial cancer,including damp heat descending type,kidney yin deficiency type,spleen deficiency and liver depression type,and dampness toxin stasis type,were 21.43%,44.29%,15.71%,and 18.57%,respectively.Among them,the proportion of patients with muscle layer infiltration depth<1/2 and≥1/2 were 64.29% and 35.71%,respectively;the proportions of patients with histological grades G1,G2,and G3 were 68.57%,17.14%,and 14.29%,respectively;the proportion of patients in stages Ⅰ,Ⅱ,and Ⅲ was 70.00%,22.86%,and 7.14%,respectively;the proportion of patients with pathological types of endometrioid adenocarcinoma and non endometrioid adenocarcinoma is 90.00%and 10.00%,respectively.Spearman’s results showed that there was a significant positive correlation(P<0.05)between the depth of muscle infiltration and the damp heat descending type(r=0.346,P<0.001),kidney yin deficiency type(r=0.582,P<0.001),spleen deficiency and liver depression type(r=0.927,P<0.001),and dampness toxin stasis type(r=0.559,P<0.001),while there was no correlation(P>0.05)between pathological types and the four Traditional Chinese Medicine syndrome types.Conclusion The TCM syndrome type of endometrial carcinoma is related to the depth of muscle infiltration,histological grade and stage.TCM syndrome type can be used as
