摘 要:目的 分析郑州市7~9岁儿童上颌第一恒磨牙异位萌出情况和相关因素。方法 回顾性分析郑州市110名7~9岁儿童的一般资料,依据《上颌第一恒磨牙异位萌出临床诊疗专家共识》将其分为异位萌出组和非异位萌出组。比较两组的一般资料、口腔检查情况;采用多因素Logistic回归分析影响儿童上颌第一恒磨牙异位萌出的独立因素;经受试者工作特征(ROC)曲线评估模型的临床价值。结果 110名儿童中,12名出现上颌第一恒磨牙异位萌出,其中8名能够实现自我矫正,4名是不可逆类型。多因素Logistic回归分析结果显示,性别、年龄、母亲学历、上颌骨发育情况、上颌牙弓长度、第二乳磨牙牙根吸收程度是儿童上颌第一恒磨牙异位萌出的独立影响因素(P<0.05)。ROC曲线分析结果显示,经构建风险评估模型,联合预测的曲线下面积(AUC)值最大,为0.992,且灵敏度、特异度分别为100.00%、96.94%。结论 儿童上颌第一恒磨牙异位萌出的影响因素为性别、年龄、母亲学历、上颌骨发育情况、上颌牙弓长度、第二乳磨牙牙根吸收程度。Objective To analyze the ectopic eruption and related factors of maxillary first permanent molars in children aged 7-9 years in Zhengzhou city.Methods The general data of 110 children aged 7-9 years in Zhengzhou city were retrospectively analyzed.According to Consensus on Clinical Diagnosis and Treatment of Ectopic Eruption of Maxillary First Permanent Molars,the children were divided into ectopic eruption group and non-ectopic eruption group.The general data and oral examination of the two groups were compared;multivariate Logistic regression analysis was used to analyze the independent factors affecting the ectopic eruption of maxillary first permanent molars in children;the clinical value of the model was evaluated by receiver operating characteristic(ROC)curve.Results Among the 110 children,12 cases had ectopic eruption of maxillary first permanent molars,of which 8 cases could achieve self-correction and 4 cases were irreversible.Multivariate Logistic regression analysis results showed that gender,age,mother's education,maxillary development,maxillary arch length and root resorption of the second deciduous molar were independent influencing factors for ectopic eruption of maxillary first permanent molars in children(P<0.05).The results of ROC curve analysis showed that the area under the curve(AUC)of the risk assessment model of combined prediction was the highest,which was 0.992,and the sensitivity and specificity were 100.00%and 96.94%,respectively.Conclusion The influencing factors of ectopic eruption of maxillary first permanent molars in children are gender,age,mother's education,maxillary development,maxillary arch length and root resorption of second deciduous molar.
