双倍剂量恩替卡韦治疗HBV相关慢加急性肝衰竭的短期疗效研究

摘　　要：目的观察双倍剂量恩替卡韦治疗HBV相关慢加急性肝衰竭的短期疗效。方法回顾性分析在本院住院的HBV相关慢加急性肝衰竭患者共90例,其中45例患者在内科保守治疗的基础上给予常规的1片(0.5mg)恩替卡韦抗病毒治疗,作为对照组;另外45例进行常规内科保守治疗的同时予以2片(1.0mg)恩替卡韦抗病毒治疗,作为实验组;收集两组患者治疗前、治疗2周、4周、8周的总胆红素、凝血酶原时间、HBV-DNA载量及Child-Pugh评分等相关数据,统计分析两组患者治疗不同时间节点的上述指标差异。结果两组患者治疗前的总胆红素水平、凝血酶原时间、病毒量以及Child-Pugh评分无明显差异;在治疗2周、4周、8周不同时间点,上述指标均存在明显差异,且实验组疗效优于对照组。结论双倍剂量的恩替卡韦对HBV相关慢加急性肝衰竭患者疗效显著,值得临床推广尝试。Objective To analyze the short-term efficacy of double dose entecavir in the treatment of HBV related acute on chronic liver failure.Methods A total of 90 patients with HBV related acute on chronic liver failure in our hospital were retrospectively analyzed.Among them,45 patients were given conventional 1 tablet(0.5mg)entecavir antiviral treatment on the basis of medical conservative treatment,as the control group.Another 45 patients were treated with conventional medical conservative treatment and 2tablets(1.0mg)of entecavir as experimental group.The data of total bilirubin,prothrombin time,HBV-DNA load and child Pugh score before treatment,2 weeks,4 weeks and 8 weeks after treatment were collected and statistically analyzed.Results There was no significant difference in total bilirubin level,prothrombin time,viral load and child Pugh score between the two groups before treatment;In the treatment of 2 weeks,4 weeks,8 weeks at different time points,the above indicators were significantly different,and the effect of the experimental group was better than that of the control group.Conclusion Double dose of entecavir in the treatment of patients with HBV related acute on chronic liver failure has significant curative effect and good safety,which is worthy of clinical promotion.
