探讨宫颈癌术后出现早期淋巴转移的影响因素

摘　　要：目的探讨社区宫颈癌发生早期淋巴转移的影响因素方法选取全区2016年-2024年筛查后随访的179例宫颈癌患者进行回顾性分析,采用单因素差异性分析及多因素Logistic回归模型进行影响因素分析.结果二元Logistic回归模型结果显示临床分期Ⅳ期(β=2.264,OR=9.621,P=0.014)、发生淋巴肿大(β=1.727,OR=5.625,P=0.009)、间质浸润深度1/3-2/3(β=0.959,OR=2.608,P=0.004)、间质浸润深度>2/3(β=0.983,OR=2.672,P=0.026)和淋巴细胞比例高(β=2.632,OR=13.897,P<0.001)是肿瘤转移的影响因素.结论临床分期Ⅳ期、发生淋巴肿大、间质浸润深度高和淋巴细胞比例高是引起肿瘤转移的危险因素,需要临床医护人员重点关注.Objective The paper explored influencing factors for early lymphatic metastasis after cervical cancer surgery.Methods 179 cervical cancer patients who were screened and followed up in the district from 2016 to 2024 were reviewed,single factor differential analysis and multivariate Logistic regression models were used to analyze the influencing factors.Results Binary Logistic regression model showed that clinical stage IV(β=2.264,OR=9.621,P=0.014),Lymph node enlargement(β=1.727,OR=5.625,P=0.009),Depth of interstitial infiltration is 1/3-2/3(β=0.959,OR=2.608,P=0.004),Depth of interstitial infiltration>2/3(β=0.983,OR=2.672,P=0.026)and the high ratio of lymphocytes(β=2.632,OR=13.897,P<0.001)are influencing factors for lymphatic metastasis.Conclusion Clinical stage IV,lymph node enlargement,depth of interstitial infiltration,and high lymphocyte ratio are risk factors for lymphatic metastasis which require special attention.
