白内障手术联合房角分离治疗闭角性青光眼合并白内障的临床效果

摘　　要：目的探讨闭角性青光眼合并白内障患者采用白内障手术联合房角分离治疗的临床效果。方法随机选取2020年6月—2023年6月泗阳朝聚眼科医院收治的闭角性青光眼合并白内障患者90例,依据差异性治疗方法分为两组,对照组(n=45)实施白内障超声乳化+人工晶体植入术+小梁切除术+周边虹膜切除术治疗;研究组(n=45)实施白内障超声乳化+人工晶体植入术联合房角分离治疗。比较两组患者术后视力、矫正视力、眼压、房角、中央前房深度改变情况。结果研究组术后视力(0.54±0.11),矫正视力(0.71±0.07)、眼压(11.05±1.02)mmHg、房角(225.64±8.68)°、中央前房深度(2.84±0.24)mm,均优于对照组的(0.46±0.09)、(0.64±0.04)、(17.64±2.15)mmHg、(204.52±6.24)°、(2.34±0.22)mm,差异均有统计学意义(t=3.776,5.824,18.577,13.253,10.302;P均<0.05)。结论闭角型青光眼采取白内障手术联合房角分离手术治疗可提高患者视力水平,降低眼压,同时可以改善房角及中央前房深度。Objective Discusses close Angle glaucoma with cataract were treated by joint corner separation treatment of the clinical effect of cataract surgery.Methods A total of 90 patients with angularity glaucoma complicated with cataract treated in Siyang Chaoju Eye Hospital from June 2020 to June 2023 were randomly selected and divided into two groups according to different treatment methods.The control group(n=45)was treated with cataract phacoemulsification+intraocular lens implantation+trabeculectomy+peripheral iridectomy.The study group(n=45)underwent cataract phacoemulsification+intraocular lens implantation combined with atrial Angle separation.The changes of postoperative visual acuity,corrected visual acuity,intraocular pressure,atrial angle and central anterior chamber depth were compared between the two groups.Results The postoperative uncorrected visual acuity(0.54±0.11),corrected visual acuity(0.71±0.07),intraocular pressure(11.05±1.02)mmHg,anterior chamber angle(225.64±8.68)°,and central anterior chamber depth(2.84±0.24)mm in the study group were all better than those in the control group(0.46±0.09),(0.64±0.04),(17.64±2.15)mmHg,(204.52±6.24)°,and(2.34±0.22)mm,respectively,the differences were statistically significant(t=3.776,5.824,18.577,13.253,10.302;all P<0.05).Conclusion Cataract surgery combined with atrial angle separation surgery can improve the visual acuity,reduce the intraocular pressure,and improve the angle and central anterior chamber depth of angle-closed glaucoma.
