摘 要:目的探讨重复经颅磁刺激(repetitive transcranial magnetic stimulation,rTMS)联合运动功能训练在脑卒中后偏瘫肢体功能康复中的疗效。方法方便选取2022年1月—2024年1月厦门市第五医院收治的68例脑卒中后偏瘫患者为研究对象,根据不同治疗方法分为两组,各34例。对照组实施运动功能训练,观察组实施rTMS联合运动功能训练。比较两组的肢体运动功能、功能独立性、日常生活能力、平衡功能、表面肌电图、步行能力。结果治疗后,观察组的上肢功能评分为(46.55±3.71)分、下肢功能评分为(26.55±3.65)分,高于对照组的(43.69±2.94)分、(23.23±2.85)分,差异均有统计学意义(t=3.523,4.180;P均<0.05)。治疗后,与对照组比,观察组平衡功能和步行能力更优,功能独立性与日常生活能力及积分肌电图更高,均方根肌电图更低,差异均有统计学意义(P均<0.05)。结论rTMS联合运动功能训练可提高单一运动功能训练在脑卒中后偏瘫肢体功能康复中的疗效,对患者的肢体运动功能、平衡功能和步行能力的改善效果较好,能使患者的生活能力进一步提高。Objective To investigate the efficacy of repetitive transcranial magnetic stimulation(rTMS)combined with motor function training in the rehabilitation of hemiplegic limb function after stroke.Methods A total of 68 patients with hemiplegia after stroke admitted to the Xiamen Fifth Hospital from January 2022 to January 2024 were conveniently selected as the research objects.They were divided into two groups,with 34 cases in each group,based on different treatment methods.The control group received motor function training,and the observation group received rTMS combined with motor function training.The limb motor function,functional independence,daily living ability,balance function,surface electromyography,walking ability were compared between the two groups.Results After treatment,the observation group had higher upper limb function score as(46.55±3.71)points and lower limb function score as(26.55±3.65)points than(43.69±2.94)points and(23.23±2.85)points in the control group,and the differences were statistically significant(t=3.523,4.180;both P<0.05).After treatment,compared with control group,the balance function and walking ability of the observation group were better,the functional independence,daily living ability and integral electromyography were higher,and the root mean square electromyography was lower,and the differences were statistically significant(all P<0.05).Conclusion rTMS combined with motor function training can improve the efficacy of single motor function training in the rehabilitation of hemiplegic limb function after stroke.It has a better effect on the improvement of limb motor function,balance function and walking ability of patients,and can further improve the living ability of patients.
