摘  要:慢加急性肝衰竭(ACLF)是一种病因复杂且病死率高的终末期肝病,预后极差,准确评估不良预后对改善患者的临床转归至关重要。ACLF患者常由于营养物质代谢紊乱而出现不同程度的营养不良。糖、氨基酸和脂质代谢对骨骼肌具有调控作用,而骨骼肌异常(如肌肉减少症和肌肉脂肪变性)是ACLF相关营养不良的重要表现,但其与ACLF预后的确切关系尚不明确。现对骨骼肌改变在ACLF预后评估中作用的最新研究进行综述,旨在为早期危险分层和治疗提供一定的临床指导。Acute-on-chronic liver failure(ACLF)is a type of end-stage liver disease characterized by complex etiology and high mortality,with an extremely poor prognosis.Accurate assessment of adverse outcomes is crucial for improving the clinical outcomes of patients.ACLF patients often experience varying degrees of malnutrition due to metabolic disturbances of nutrients.The metabolism of glucose,amino acids,and lipids plays a regulatory role in skeletal muscle,and skeletal muscle abnormalities(such as sarcopenia and myosteatosis)are important manifestations of malnutrition associated with ACLF.However,the exact relationship between these abnormalities and the prognosis of ACLF remains unclear.This article reviews the latest research on the role of skeletal muscle changes in the prognostic evaluation of ACLF,aiming to provide clinical guidance for early risk stratification and treatment.
