阿替普酶静脉溶栓治疗急性脑梗死的临床效果分析

摘　　要：目的:探讨阿替普酶(rt-PA)静脉溶栓治疗急性脑梗死(ACI)的临床效果。方法:选取2019年9月—2021年8月菏泽市成武县人民医院收治的ACI患者100例为研究对象,随机分为对照组与研究组,各50例。对照组采用尿激酶溶栓治疗,研究组采用rt-PA静脉溶栓治疗。比较两组神经功能、血流动力学指标水平、临床疗效、不良反应发生情况。结果:治疗前,两组美国国立卫生研究院卒中量表评分比较,无统计学差异(P>0.05);治疗后,两组美国国立卫生研究院卒中量表评分降低,研究组低于对照组(P<0.05)。治疗前,两组全血黏度、红细胞聚集指数比较,无统计学差异(P>0.05);治疗后,两组全血黏度、红细胞聚集指数降低,研究组低于对照组(P<0.05)。研究组治疗总有效率高于对照组(P=0.032)。两组不良反应总发生率比较,无统计学差异(P>0.05)。结论:rt-PA静脉溶栓治疗ACI的效果显著,可改善患者神经功能及血流动力学指标,且安全性较高。Objective:To investigate the clinical effect of Alteplase(rt-PA)intravenous thrombolytic therapy on acute cerebral infarction(ACI).Methods:A total of one hundred patients with ACI treated in Heze Chengwu County People's Hospital from September 2019 to August 2021 were selected as the study objects and randomly divided into control group and study group,with fifty patients in each group.The control group was treated with urokinase thrombolytic therapy,and the study group was treated with rt-PA intravenous thrombolytic therapy.Neurological function,hemodynamic index level,clinical efficacy and adverse reactions were compared between the two groups.Results:Before treatment,there was no significant difference in the score of national institutes of health stroke scale between the two groups(P>0.05).After treatment,the national institutes of health stroke scale score decreased in both groups,and the study group was lower than the control group(P<0.05).Before treatment,there were no significant differences in whole blood viscosity and red blood cell aggregation index between the two groups(P>0.05).After treatment,the whole blood viscosity and red blood cell aggregation index of the two groups were decreased,and the study group was lower than the control group(P<0.05).The total effective rate of the study group was higher than that of the control group(P=0.032).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Rt-PA intravenous thrombolysis is effective in the treatment of ACI,which can improve the neurological function and hemodynamic indexes of patients with high safety.
