以肌无力为首发症状的甲状旁腺功能亢进症1例报告

摘　　要：本文报道1例以肌无力为首发症状的甲状旁腺功能亢进症病例。患者女性,48岁,因全身乏力在当地医院诊断为“中枢神经系统脱髓鞘疾病”,经治疗后未见好转。随后出现小便失禁,就诊于遵义医科大学附属医院,实验室检查提示甲状旁腺功能亢进、高钙血症,诊断甲状旁腺功能亢进症伴肌无力。纠正高钙血症后予手术治疗,术后病理示甲状旁腺腺瘤,复查甲状旁腺激素、血钙、肌力恢复正常。对于此类患者,需警惕由甲状旁腺腺瘤引起的甲状旁腺功能亢进伴肌无力,在明确诊断后,应及时进行手术切除治疗。A female patient of 48 years old was reported for hyperparathyroidism with myasthenia as initial symptom.The patient was initially diagnosed of“central nervous system demyelinating disease”in the local hospital due to general fatigue,and was not improved after treatment,who subsequently had urinary incontinence and was admitted in the Affiliated Hospital of Zunyi Medical University.The laboratory tests revealed that the patient had hyperparathyroidism and hypercalcemia,and was thus diagnosed as hyperparathyroidism with myasthenia.After hypercalcemia was treated,the patient underwent a surgery with the pathology of parathyroid adenoma.The parathyroid hormone,blood calcium and muscle strength returned to normal after the operation.For the patients with hyperparathyroidism manifested with myasthenia,parathyroid tumor should be one of the considerations and the resection should be timely performed.
