摘　　要：目的在予基础滴眼液治疗的同时,比较0.05%环孢素滴眼液与氯替泼诺妥布霉素滴眼液治疗春季卡他性角结膜炎的效果。方法选取2020年7月至2021年6月于南昌大学第二附属医院眼科门诊就诊的48例春季卡他性角结膜炎患者作为研究对象。根据患者治疗意愿,将其分为治疗组(24例,48眼)和对照组(24例,48眼)。2组均予盐酸奥洛他定和玻璃酸钠滴眼液治疗,治疗组联合使用0.05%环孢素滴眼液,对照组联合使用氯替泼诺妥布霉素滴眼液,治疗周期均为60 d。比较2组在治疗前、治疗后(第7、15、30、60天)体征评分和自觉症状评分的变化,并记录药物不良反应的发生情况。结果治疗后,2组患者的体征评分和自觉症状评分均低于其治疗前(均P<0.05)。治疗后第15天和第30天,治疗组球结膜充血水肿评分低于对照组(均P<0.05);治疗后第30天,治疗组上睑结膜巨大乳头评分低于对照组(P=0.035);在其他随访时间点,2组的体征评分与自觉症状评分比较差异无统计学意义(均P>0.05)。对照组出现2例眼压升高,治疗组未见不良反应发生。结论与氯替泼诺妥布霉素滴眼液治疗春季卡他性角结膜炎相比,0.05%环孢素滴眼液具有相似疗效,并能避免激素类药物的不良反应。Objective While receiving basic eye drops treatment,to compare the efficacy of 0.05%cyclosporine A eye drops and loteprednol etabonate tobramycin eye drops in the treatment of vernal keratoconjunctivitis(non-randomized controlled study).Methods A total of 48 patients diagnosed with vernal keratoconjunctivitis who visited the ophthalmology outpatient department of the Second Affiliated Hospital of Nanchang University between July 2020 and June 2021 were selected for the study.Based on the patients’treatment preferences,they were divided into the treatment group(24 cases,48 eyes)and the control group(24 cases,48 eyes).The treatment group used 0.05%cyclosporine A eye drops,while the control group used loteprednol etabonate tobramycin eye drops for treatment.Both groups were treated with a combination of olopatadine hydrochloride eye drops and sodium hyaluronate eye drops,with a treatment duration of 60 days.The changes in sign scores and subjective symptom scores before and after treatment(at 7,15,30,and 60 days)were compared between the two groups,and adverse drug reactions were recorded.Results After treatment,both groups showed significantly lower sign and symptom scores compared to pre-treatment(both P<0.05).On days 15 and 30,the treatment group had significantly lower conjunctival congestion and edema scores than the control group(both P<0.05).On day 30,the treatment group had significantly lower upper tarsal conjunctival giant papillae scores than the control group(P=0.035).No significant differences were observed in sign and symptom scores between the two groups at other follow-up time points(all P>0.05).Two cases of increased intraocular pressure were reported in the control group,while no adverse drug reactions occurred in the treatment group.Conclusion Compared with loteprednol etabonate tobramycin eye drops,0.05%cyclosporine A eye drops show similar efficacy in the treatment of vernal keratoconjunctivitis and can effectively avoid the adverse effects of steroidal drugs.
