摘　　要：目的 探讨ICU老年患者认知衰弱的发生情况及其独立影响因素。方法 2023年10月—2024年5月,采用便利抽样法选取南华大学附属长沙中心医院545例ICU老年患者为研究对象。采用一般资料调查表、急性生理及慢性健康状况评分Ⅱ、ICU意识模糊评估量表、理查兹—坎贝尔睡眠量表、简版老年抑郁量表和社会支持评定量表收集数据。通过单因素分析和相关性分析筛选ICU老年患者认知衰弱发生的影响因素,并采用二元logistic回归分析探讨ICU老年患者认知衰弱发生的独立影响因素。结果 ICU老年患者认知衰弱的发生率为39.6%。logistic回归分析显示,年龄、急性生理及慢性健康状况评分、抑郁、谵妄发生次数、有创通气时间、社会支持度及早期活动为ICU老年患者认知衰弱发生的独立影响因素(P均<0.05)。结论 ICU老年患者认知衰弱发生率较高。年龄、疾病严重程度、抑郁、谵妄次数、有创通气时间、社会支持水平和早期活动对认知衰弱的发生产生影响,应针对上述因素加强监测与干预,以降低ICU老年患者的认知衰弱发生风险。Objective To explore the incidence and independent influencing factors of cognitive frailty in elderly ICU patients.Methods From October 2023 to May 2024,a convenience sampling method was used to select 545 elder-ly ICU patients from the Affiliated Changsha Central Hospital of University of South China as the study subjects.Data were collected using a general information survey,Acute Physiology and Chronic Health EvaluationⅡ(APACHEⅡ),ICU De-lirium Screening Checklist,Richards-Campbell Sleep Questionnaire,Geriatric Depression Scale(short version),and So-cial Support Rating Scale.Univariate analysis and correlation analysis were used to screen the influencing factors of cogni-tive frailty in elderly ICU patients,and binary logistic regression was used to explore the independent influencing factors of cognitive frailty in elderly ICU patients.Results The incidence of cognitive frailty in elderly ICU patients was 39.6%.Logistic regression analysis showed that age,APACHEⅡscore,depression,frequency of delirium,invasive ventilation time,social support,and early mobilization were independent influencing factors for the occurrence of cognitive frailty in elderly ICU patients(all P<0.05).Conclusions The incidence of cognitive frailty in elderly ICU patients is high.Age,severity of illness,depression,frequency of delirium,invasive ventilation time,social support level,and early mobiliza-tion affect the development of cognitive frailty.Monitoring and intervention targeting these factors should be strengthened to reduce the risk of cognitive frailty in elderly ICU patients.
