非典型抗精神病药物联合丙戊酸钠治疗双相躁狂症患者的作用研究

摘　　要：目的探究非典型抗精神病药物联合丙戊酸钠治疗双相躁狂症患者的效果。方法选取2020年5月至2022年8月吉安市第三人民医院收治的82例双相躁狂症患者作为研究对象,按照随机抽签法分为对照组(予以丙戊酸钠治疗)与观察组(予以非典型抗精神病药物、丙戊酸钠联合治疗),每组41例。比较两组临床疗效、炎症因子水平、贝克-拉范森躁狂量表(Bech-Rafaelsen mania rating scale,BRMS)评分及不良反应发生率。结果观察组治疗总有效率为95.12%,高于对照组的78.05%,差异有统计学意义(P<0.05)。用药8周后,两组血清白细胞介素-1β(interleukin-1β,IL-1β)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平均低于用药前,白细胞介素-10(interleukin-10,IL-10)水平高于用药前,且观察组IL-1β、TNF-α水平均低于对照组,IL-10水平高于对照组,差异有统计学意义(P<0.05)。用药8周后,两组BRMS评分均低于用药前,且观察组低于对照组,差异有统计学意义(P<0.05)。两组用药期间不良反应发生率比较差异无统计学意义。结论非典型抗精神病药物联合丙戊酸钠治疗双相躁狂症的效果更理想,利于控制患者机体炎症反应,改善躁狂症状,促进病情恢复,且安全性较高,值得临床推广应用。Objective To explore the effect of atypical antipsychotics combined with sodium valproate in the treatment of patients with bipolar mania.Methods 82 patients with bipolar disorder admitted to Ji'an Third People's Hospital from May 2020 to August 2022 were selected as the research subjects,and they were divided into the control group and the observation group,with 41 cases in each group.The control group was treated with sodium valproate,and the observation group was treated with atypical antipsychotics combined with sodium valproate.The clinical efficacy levels of inflammatory factors,Bech-Rafaelsen mania rating scale(BRMS)score and incidence of adverse reactions were compared between the two groups.Results The total effective rate in the observation group was 95.12%,which was higher than 78.05%in the control group,the difference was statistically significant(P<0.05).After 8 weeks of mediation,the serum levels of interleukin-1β(IL-1β),and tumor necrosis factor-α(TNF-α)of the two groups were lower than those before medication,while the level of interleukin-10(IL-10)was higher than that before medication,and serum levels of IL-1β,TNF-αin the observation group were lower than those in the control group,and the level of IL-10 was higher than that in the control group,the differences were statistically significant(P<0.05).After 8 weeks of mediation,the BRMS score of the two groups were lower than that before mediation,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).There was no difference in the incidence of adverse reactions between the two groups.Conclusion Atypical antipsychotics combined with sodium valproate are more effective in the treatment of bipolar mania,which is beneficial to control the inflammatory reaction of patients,improve manic symptoms and promote the recovery of the disease,and is worthy of publicity,and it is safe and worthy of clinical promotion and application.
