清胃愈疡汤联合雷贝拉唑治疗溃疡性上消化道出血的临床效果分析

摘　　要：目的:探讨清胃愈疡汤联合雷贝拉唑治疗溃疡性上消化道出血(UGIH)患者的临床疗效。方法:选取2021年8月—2023年8月广东省中西医结合医院沙头院区收治的96例溃疡性UGIH患者为研究对象,采用随机数字表法将其分为研究组和对照组,各48例。对照组接受雷贝拉唑治疗,研究组接受清胃愈疡汤联合雷贝拉唑治疗,两组均治疗2个月。比较两组临床疗效、中医证候积分、血清指标、胃酸强度及不良反应发生情况。结果:研究组治疗总有效率为95.83%高于对照组的79.19%,差异有统计学意义(P<0.05)。治疗后,研究组中医证候各项积分均低于对照组,差异有统计学意义(P<0.05)。研究组白细胞介素-6水平低于对照组,血红蛋白水平、空腹胃酸pH值均高于对照组,差异均有统计学意义(P<0.05)。研究组并发症发生率为12.50%,对照组并发症发生率为6.25%,差异无统计学意义(P>0.05)。结论:清胃愈疡汤联合雷贝拉唑治疗溃疡性UGIH疗效显著,能有效改善临床症状,减轻炎症反应,改善胃酸PH,从而促进患者恢复,且联合用药不增加安全性风险。Objective:To explore the clinical curative effect of combined treatment with Qingwei Yuyang Decoction and Rabeprazole on patients with ulcerative upper gastrointestinal hemorrhage(UGIH).Methods:A total of 96 patients with ulcerative UGIH admitted to the Shatou Hospital of Guangdong Provincial Hospital of Integrated Traditional an Western Medicine hospital from August 2021 to August 2023 were selected as the research subjects.They were randomly divided into an study group and a control group,with 48 cases in each group,using a random number table method.The control group and the study group were treated with Rabeprazole and Qingwei Yuyang Decoction combined with rabeprazole,respectively.Two months later,clinical effects,traditional Chinese medicine syndrome scores,serum indices,gastric acidity,and the accurrence of adverse reactions were compared between the 2 groups.Results:The total effective rate of the study group was 95.83%,which was higher than 79.19%of the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of traditional Chinese medicine syndrome in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).the level of interleukin-6 in the study group was lower than that in the control group,while the levels of hemoglobin and fasting gastric acid pH were higher than those in the control group,with statistically significant differences(P<0.05).The incidence rate of complications in the study group and the control study group were 12.50%and 6.25%,there was no statistically significant difference(P>0.05).Conclusion:Qingwei Yuyang Decoction combined with Rabeprazole is significantly effective in the treatment of ulcerative UGIH.It can effectively improve clinical symptoms,alleviate inflammatory reactions,and improve the pH value of gastric acid,thereby promoting recovery.Moreover,the combined treatment does not increase safety risk.
