痔上黏膜环切术联合补中益气汤加减治疗直肠内脱垂患者的效果分析

摘　　要：目的:探讨痔上黏膜环切术(PPH)联合补中益气汤加减对直肠内脱垂(IRP)患者的临床疗效。方法:选取2023年6—12月淄博市张店区中医院收治的78例IRP患者为研究对象,根据随机数字表法将其分为对照组和研究组,各39例。对照组采用PPH治疗,研究组在对照组基础上联合补中益气汤加减治疗。两组均治疗7 d。对比两组的炎症指标、中医证候积分、生活质量、并发症发生情况。结果:术后,两组超敏C反应蛋白、肿瘤坏死因子-α及白细胞介素-6水平均较术前降低,且研究组均低于对照组,差异均有统计学意义(P<0.05)。术后,两组排便困难、肛门坠胀感、肛门失禁和肛周瘙痒的中医证候积分均较术前降低,且研究组均低于对照组,差异均有统计学意义(P<0.05)。术后,两组大便失禁生活质量表中各个维度评分均较术前升高,且研究组均高于对照组,差异均有统计学意义(P<0.05)。研究组的并发症发生率20.51%低于对照组的5.13%,差异有统计学意义(P<0.05)。结论:PPH结合补中益气汤加减治疗IRP患者相较于单纯PPH治疗,能够更有效地降低炎性因子水平,改善中医证候积分,提高生活质量,减少并发症。Objective:To investigate the clinical effect of procedure for prolapse and hemorrhoid(PPH)combined with Buzhong Yiqi Decoction Add and Minus in the treatment of internal rectal prolapse(IRP).Methods:A total of 78 cases of IRP patients admitted to Zhangdian District Hospital of Traditional Chinese Medicine in Zibo City from June to December 2023 were selected as the study objects,and were divided into a control group and a study group according to random number table method,with 39 cases in each group.The control group was treated with PPH,and the study group was treated with Buzhong Yiqi Decoction Add and Minus on the basis of the control group The inflammatory indicators,TCM syndrome scores,quality of life,and occurrence of complications.Two groups were treated 7 d.The therapeutic effect of the two groups was compared.Results:After treatment,the levels of hypersensitive C-reactive protein,tumor necrosis factor-αand interleukin-6 in two groups were lower than before treatment,and the study group were lower than the control group,the differences were statistically significant(P<0.05).After treatment,the TCM syndrome scores of defecation difficulty,anal distension,anal incontinence and perianal pruritus in both groups were lower than before treatment,and the study group were lower than the control group,with statistical significance(P<0.05).After treatment,the scores of all dimensions of the Fecal Incontinence Quality of Life Scale in both groups were higher than before treatment,and the study group was higher than the control group,the differences were statistically significant(P<0.05).The complication rate of 20.51%in the study group,which was lower than 5.13%in the control group,the difference was statistically significant(P<0.05).Conclusion:PPH combined with Buzhong Yiqi Decoction Add and Minus can effectively reduce the level of inflammatory factors,improve TCM syndrome scores,improve quality of life and reduce complications in IRP patients compared with PPH alone.
