以皮肤损害为首发表现的结外NK/T细胞淋巴瘤1例

摘　　要：患者男,70岁,左侧面部红斑9个月余,溃疡半个月。患者9个月前因左侧颧颊部被蚊虫叮咬后出现红斑、丘疹伴瘙痒,多次至当地医院行抗感染治疗,治疗效果不佳,上述皮疹面积逐渐增大,入院前半个月红斑中央出现破溃。皮损组织病理示:真皮和皮下组织见淋巴细胞弥漫性增生浸润,肿瘤细胞以血管为中心弥漫浸润,破坏血管壁,侵犯周围组织,肿瘤细胞胞体大,异型性明显,可见核仁,染色质丰富。免疫组织化学染色示:CD3/4/8/20/30/56/68(+),TIA(+),GrB(+),BCL2(散在+),EBER(+),Ki67(约20+)。诊断:结外NK/T细胞淋巴瘤。予以CHOP方案治疗,皮疹较前明显好转。A 70-year-old male had erythema on his left face for more than 9 months and ulcers for half a month.The patient developed erythematous papules and itching after being bitten by a mosquito on the left cheek 9 months ago.He went to the local hospital many times for anti-infective treatment,but the treatment effect was not good.The area of the rash gradually increased,and a rupture appeared in the center of the erythema half a month before admission.Histopathology of the skin lesions showed diffuse proliferation and infiltration of lymphocytes in the dermis and subcutaneous tissue.Tumor cells diffusely infiltrated around blood vessels,destroying the blood vessel walls and invading surrounding tissues.The tumor cell bodies were large,with obvious atypia,nucleoli and rich chromatin.Immunohistochemical staining:CD3/4/8/20/30/56/68(+),TIA(+),GrB(+),BCL2(scattered+),EBER(+),Ki67(about 20+).The diagnosis was extranodal NK/T cell lymphoma.And the rash improved significantly with the treatment of CHOP regimen.
