皮肤纤维瘤伴皮脂腺诱导1例

摘　　要：患者男,47岁,右肩部皮疹10余年。皮肤科情况:右肩部一花生米大小斑块,表面略粗糙,中央见米粒大小黄红色丘疹,边缘伴环形色素沉着。皮损组织病理示:表皮角化过度,棘层肥厚,表皮突下延,真皮内见由相互交错呈席纹状或旋涡状梭形细胞束和胶原束组成的肿瘤团块,境界欠清,靠近表皮处可见密集分布的成熟皮脂腺小叶。结合临床表现和组织病理,诊断为皮肤纤维瘤伴皮脂腺诱导。治疗:手术全切,随访6个月未复发。A 47-year-old man presented with a rash on the right shoulder for more than 10 years.Dermatological examination showed a peanut-sized plaque on the right shoulder with a slightly rough surface,and a yellow-red papule of rice grain size was seen in the center,surrounded by annular pigmentation.Histopathology of skin lesions revealed hyperkeratosis,acanthosis,and the downward extension of epidermal rete ridges.In the dermis,a well-circumscribed tumor mass was observed,composed of interwoven fascicles of fusiform cells and collagen fibers arranged in a storiform or whorled pattern.Dense mature sebaceous gland lobules were observed adjacent to the epidermal layer.According to the clinical manifestations and histopathological examination,the patient was diagnosed with dermatofibroma with sebaceous induction.Treatment involved complete surgical excision,and no recurrence was found after 6 months of follow-up.
