微波原位灭活联合病灶内刮除术治疗长骨非典型软骨肿瘤的疗效分析

摘  要:目的探讨微波原位灭活联合病灶内刮除术治疗长骨非典型软骨肿瘤的的临床疗效。方法回顾分析2014年4月至2019年8月在我院治疗的19例四肢长骨非典型软骨肿瘤患者资料、术前准备、手术技术、术后随访及并发症情况。结果截止2024年1月,本组19例随访38~116个月,平均74个月。1例术后8个月复发,术后38个月死于肺转移。1例术后4个月手术部位骨折。除去1例死亡者,其余18例的骨骼肌肉系统肿瘤协会(musculoskeletal tumor society,MSTS)评分为20~30分(67%~100%),平均27分(90%)。结论微波原位灭活联合病灶内刮除术治疗长骨非典型软骨肿瘤复发率低,并发症发生率低,肢体功能良好,临床疗效满意。Objective To investigate the clinical efficacy of microwave in situ inactivation combined with intralesional curettage in the treatment of atypical cartilaginous tumors.Methods Preoperative preparation,surgical techniques,postoperative follow-up and complications of 19 patients with atypical cartilage tumors of the long bones of the limbs treated in our hospital from April 2014 to August 2019 were retrospectively analyzed.Results As of January 2024,patients in this group were followed up for 38-116 months,with an average of 74 months.One patient experienced recurrence 8 months after surgery and died of lung metastasis 38 months after surgery.One patient suffered a surgical site fracture 4 months after surgery.Except for one deceased patient,the MSTS scores of the other 18 patients were 20-30 points(67%-100%),with an average of 27 points(90%).Conclusions Microwave in situ inactivation combined with intralesional scraping surgery has low recurrence rate,low incidence of complications,good limb function,and satisfactory clinical efficacy for the treatment of atypical cartilage tumors of long bones.
