摘 要:目的 探究消化性溃疡合并幽门螺杆菌(Hp)感染患儿长链非编码核糖核酸(LncRNA)H19、微小核糖核酸(miR)-625-5p、miR-26a水平,及各指标与炎性指标的相关性。方法 选取海南省妇女儿童医学中心2023年1-12月收治的63例消化性溃疡合并Hp感染患儿(阳性组,Ⅰ型38例、Ⅱ型25例),77例消化性溃疡未合并Hp感染(阴性组),另收集同期155名体检健康儿童为健康对照组。比较健康对照组、阳性组、阴性组之间及Ⅰ型组、Ⅱ型组之间肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-6、IL-8、IL-18、IL-2、IL-33及LncRNA H19、miR-625-5p、miR-26a水平,采用Pearson相关性分析LncRNA H19、miR-625-5p、miR-26a与炎性指标的相关性。结果 阳性组TNF-α、IL-6、IL-8、IL-18、IL-33及LncRNA H19、miR-26a分别为(22.32±4.77) ng/ml、(50.37±11.79)pg/ml、(47.75±10.91)pg/ml、(12.27±3.09)pg/ml、(66.24±12.56)pg/ml、(2.85±0.85)、(3.24±0.97)均高于阴性组、健康对照组,阳性组高于阴性组(P<0.05),阳性组、阴性组IL-2、miR-625-5p水平低于健康对照组,且阳性组低于阴性组(P<0.05)。Ⅰ型组LncRNA H19、miR-26a、TNF-α、IL-6、IL-8、IL-18、IL-33水平均高于Ⅱ型组,miR-625-5p、IL-2水平低于Ⅱ型组(P<0.05)。LncRNA H19和miR-26a与TNF-α、IL-6、IL-8、IL-18、IL-33水平呈正相关(r=0.737、0.685、0.629、0.608、0.623和0.783、0.618、0.673、0.690、0.627,P均<0.05),与IL-2呈负相关(r=-0.711、-0.644,P=0.015、0.006);miR-625-5p与TNF-α、IL-6、IL-8、IL-18、IL-33呈负相关(r=-0.695、-0.663、-0.631、-0.667、-0.605,P均<0.05),与IL-2呈正相关(r=0.736,P=0.013)。结论 LncRNA H19、miR-625-5p、miR-26a及炎性指标在消化性溃疡合并Hp感染患儿及不同基因型Hp感染所致消化性溃疡患儿中表达异常,且LncRNA H19、miR-625-5p、miR-26a与炎性指标水平存在相关性。OBJECTIVE To investigate the levels of long non-coding RNA(LncRNA)H19,micro ribonucleic acid(miR)-625-5p and miR-26a in children with peptic ulcer complicated by Helicobacter pylori(Hp)infection and their correlations with inflammatory indicators.METHODS Sixty-three children with peptic ulcer complicated by Hp infection with 38 cases of type I and 25 cases of type I(positive group),and 77 cases of peptic ulcer with-out Hp infection(negative group)admitted to Hainan Women and Children's Medical Center from Jan.2023 to Dec.2023 were selected,and additionally 155 healthy children underwent physical examination during the same period were collected as the healthy control group.The levels of tumor necrosis factor-α(TNF-α),interleukin(IL)-6,IL-8,IL-18,IL-2,IL-33,LncRNA H19,miR-625-5p and miR-26a were compared among the healthy control,positive and negative groups,and between the type I and II groups.Pearson correlation analysis was used to analyze the correlation between LncRNA H19,miR-625-5p and miR-26a and inflammatory indicators.RESULTS The levels of TNF-α,IL-6,IL-8,IL-18,IL-33,LncRNA H19 and miR-26a in the positive group were(22.32±4.77)ng/ml,(50.37±11.79)pg/ml,(47.75±10.91)pg/ml,(12.27±3.09)pg/ml,(66.24±12.56)pg/ml,(2.85±0.85)and(3.24±0.97),respectively,higher than those in the negative and the healthy control groups,which in the positive group was higher than those in the negative group(P<o.05).The levels of IL-2 and miR-625-5p in the positive group were the lowest,followed by the negative and healthy control group(P<0.05).The levels of LncRNA H19,miR-26a,TNF-α,IL-6,IL-8,IL-18 and IL-33 in the type I group were higher than those in the type II group,and the levels of miR-625-5p and IL-2 were lower than those in the type I group(P<0.05).LncRNA H19 and miR-26a were positively correlated with TNF-α,IL-6,IL-8,IL-18 and IL-33(r=0.737,0.685,0.629,0.608,0.623 and0.783,0.618,0.673,0.690,0.627,P<0.05,respectively)and neg atively correlated with IL-2(r=-0.711,-0.644,P=0.015,0.006,respectively).The miR-625-5p wa
