摘 要:目的比较佩戴角膜塑形镜(OK镜)和特殊设计框架眼镜对低龄近视儿童眼轴控制的效果。方法回顾性队列研究。纳入2021年9月至2023年5月在成都爱迪眼科医院配镜的低龄中低度近视儿童129例(248眼)。按不同矫正方式分为:OK镜组(60例,115眼)和特殊设计框架眼镜组(69例,133眼)(含三种设计亚组)。所有儿童在小瞳下使用生物测量仪分别测量基线及戴镜12个月后的眼轴长度,分别采用独立样本t检验和单因素方差比较OK镜组与框架镜组,及不同框架亚组间AL增长量的差异。结果戴镜12个月,OK镜组与框架镜组眼轴增长量分别为(0.27±0.17)和(0.26±0.20)mm,差异无统计学意义(t=-0.26,P=0.798)。高度非球微透镜眼镜(HAL)、多焦点正向光学离焦眼镜(DIMS)、周边视力控制技术眼镜(蔡司成长乐加强版ZMAA)三种不同设计的框架眼镜眼轴增长量分别为(0.17±0.19)、(0.29±0.22)和(0.35±0.16)mm,总体差异有统计学意义(F=13.77,P<0.001),HAL控制效果优于ZMAA、DIMS(P<0.001,P=0.007),DIMS与ZMAA眼轴年增加量差异无统计学意义(P=0.212)。结论对于低龄近视儿童佩戴OK镜与特殊设计框架眼镜对眼轴控制效果相当。Objective To compare the efficacy of orthokeratology lenses(OK lenses)and specially designed spectacle lenses for the control of axial length in low-age myopic children.Methods This was a retrospective cohort study.One hundred and twenty-nine low-age myopic children(248 eyes)who were fitted with lenses at Chengdu Aidi Eye Hospital from Sep.2021 to May 2023 were included.Participants were divided into two groups based on the methods of correction:an OK lenses group(60 cases,115 eyes)and a special spectacle lenses group(69 cases,133 eyes)(including three design subgroups).All children underwent biometric measurements under small pupil conditions to assess axial length(AL)baseline and the 12-months follow-up AL.Independent sample t-tests and one-way ANOVA were utilized to compare the changes in AL between the OK lens group and the spectacle lens group,as well as among different spectacle lens subgroups.Results At 12 months after wearing,AL growth was(0.27±0.17)mm in the OK lens group and(0.26±0.20)mm in the spectacle group,with no statistically significant difference(t=-0.26,P=0.798).AL growth was(0.17±0.19),(0.29±0.22),and(0.35±0.16)mm,resulted from the three different designs of frame glasses:highly aspheric multifocal(HAL),defocus incorporated multiple segments(DIMS),and peripheral vision control technology(ZEISS Myovision ACE Asiana ZMMA),respectively,with statistically significant differences(F=13.77,P<0.001).The control effect of HAL was superior to that of ZMAA and DIMS(P<0.001,P=0.007),while there was no statistically significant difference in the annual increase of AL between DIMS and ZMAA(P=0.212).Conclusion For low-age myopic children,the efficacy of OK lenses and specially designed spectacle lenses on AL control is comparable.
